摘　　要：目的研究组胺在幽门螺杆菌(Hp)发病中的作用。方法采用放射免疫法对58例患者的胃粘膜进行了组胺含量的测定,同时检测 Hp 及进行组织 HE 染色观察炎症情况。结果 23例十二指肠溃疡(DU)者(活动期12例,愈合期11例)Hp 阳性率为73.9%,35例慢性胃炎者Hp 阳性率为52.2%。组胺含量在 Hp 阳性 DU 与阴性患者比较,差异无显著性,但活动期 DU 者显著低于愈合期 DU,而 Hp 阳性的 DU、慢性胃炎者的组胺含量显著低于 Hp 阴性慢性胃炎者。Hp阳性的 DU 与慢性胃炎患者间比较,差异无显著性。炎症程度与组胺含量无明显关系,但在活动性炎症时明显低于非活动性炎症。结论 Hp 感染可促使胃粘膜中组胺的释放,此与胃酸分泌增加,更重要的是与组织损伤有关。Objective To study the histamine content of gastric mucosa in the pathogenetic mechanism of Helicobacter pylori (Hp).Methods A radioim- munoassay method was used to determine the histamine concentration of gastric mucosa in the patients with duodenal ulcer (n=23)and chronic gastritis (n=35).The mucosa was also examined for Hp and inflammatory status by eosin staining. Results 73.9% of the DU patients (active DU 12,healed 11) and 52.2% of the chronic gastritis patients were Hp positive.There was no significant difference in histamine concentration between Hp-positive and Hp-negative DU patients,but a significant difference was observed when histamine content of the active DU patients was compared with that of healed DU patients.Hp positive patients with and without DU had significantly lower gastric histamine concentrations than Hp-negative subjects without DU.There was no significant difference in histamine concentration between Hp positive patients with DU and those without DU.No correlation was present between histamine content and mucosal inflammatory grading,but it was lower in patients with active inflammation.Conclusions Hp infection may lead to increased histamine release from the histamine-storing cells of the gastric mucosa which induces increased gastric acid secretion and,more importantly relates to mucosal injury.
