摘　　要：目的探讨内镜下黏膜切除术(endoscopic mucosal resection,EMR)治疗大肠侧向发育型肿瘤(laterally spreading tumor,LST)的方、法效果、并发症等相关临床问题。方法经内镜检查发现132例167个LST病灶,采用规范的黏膜下注射配合透明帽辅助法手术,术后随访2个月～50个月,准确记录病灶大小、手术经过、并发症发生及处理情况、病理及随访结果。结果本组技术成功率为100%,治愈率为98.8%,一次性完整切除率73.1%,分次切除法(EPMR)占10.2%,并发症发生率9.6%.术后病理癌变4例(2.4。%结)论EMR在目前条件下是一安全有效的微创技术,可取代98%以上患者的传统外科手术治疗。Objective To explore the effect ,methods and complication Of endoscopic mucosal resection（EMR） for treatment of coloreetal laterally spreading tumor （LST）. Methods 132 patients with 167 lesions were included in this study.EMR was performed by submucosal injection of sodium or with transparent cap technique.All patients were followed up by electric colonoscopic examination for 2-50 months.The complications and following treatment were recorded .All removed mucosal pieces were reclaimed for pathological study.Results 167 lesions were removed in 132 patients. The mucosectomy rate was 100%. The total curative rate was 98.8%. The lesions of 122 （73.1%）completely removed by first EMR procedure. The others were treated by EPMR and APC.The rate of complications was 9.6%.11 cases （8.3%） occurred hemorrhage during EMR procedure ,5 patients （3.8%） occurred delayed hemorrhage .No one suffered through perforation.Conclusion EMR is a safe and effective therapy for the treatment of laterally spreading colorectal tumor. It can substitute traditional surgery operation for 98% patients with eolorectal lesion.
