摘 要:目的观察空肠输注必需氨基酸对健康犬胰腺外分泌的影响,并初步探讨其可能的激素机制。方法6只健康犬建立胰腺外分泌模型,同时行空肠、胃及膀胱造瘘,股静脉插管。模型制成24h后,隔日空肠输注必需氯基酸(实验组)和对照组。于输注前30min(110)、输注后0~30min(T1)、30~60min(T2)、60~90min(T3)、90~120min(T4)时间段分别收集胰液并测定胰液体积、胰液中淀粉酶、总蛋白和主要电解质浓度,同时检测输注前及后30、60、90、120min时点血浆胆囊收缩素(cholecyslokinin,CCK)浓度。结果空肠输注苯丙氨酸或色氨酸后犬胰液量、胰淀粉酶及胰蛋白浓度较对照组显著升高(P〈0.05),而各组之间主要电解质浓度差异不显著。与对照组比较,苯丙氨酸组CCK浓度在30、60min时点明显升高(P〈0.05),色氨酸组CCK浓度在30min时点明显升高(P〈0.05)。苯丙氨酸组和色氨酸组胰液分泌量、胰淀粉酶及胰蛋白浓度改变与CCK浓度变化一致。结论空肠输注苯丙氨酸或色氨酸可改变胰液分泌,其机制可能与升高的CCK浓度刺激胰腺分泌有关。Objective To investigate the effect of jejunal perfusion of essential amino acids on pancreatic exocrine secretion in dogs. Methods The pancreatic juice external drainage was established in 6 healthy adult dogs; stomach, jejunum, bladder fistula and femoral vein intubation were also performed. Solution of 6 amino acids (experimental groups) or normal saline (control group) were infused into the jejunum of six dogs on alternative days 24 h after model established. The draining volume of pancreatic juice and the levels of amylase, total protein, Na^+, K^+, Cl^-, Ca^2+ and Mg^2+ in pancreatic juice were determined 30min before, 0-30min, 30-60min, 60-90min and 90-120min during the infusion. The plasma levels of cholecystokinin (CCK) were also measured at the start of infusion (0 min), and 30, 60, 90, and 120min during infusion. Results The draining volume, the levels of amylase and protein contents of pancreatic juice in phenylalanine group and tryptophane group were significant higher than those in control group (P〈0.05), and for of Na^+, K^+, Cl^-, Ca^2+ and Mg^2+ levels in pancreatic juice there were no significant differences (P〉0.05). The plasma CCK levels in the phenylalanine group at 30 and 60 rain and in tryptophane group at 30 min were higher than those in control group (all P〈0.05). The changes of draining volume, amylase and protein contents of pancreatic juice were correlated with the increasing of plasma CCK levels in phenylalanine and tryptophane groups. Conclusion Jejunal infusion of phenylalanine and tryptophane can increase the draining volume, the amylase and protein contents of pancreatic juice, which is associated with plasma CCK level changes.
