侧脑室三角区肿瘤顶内沟入路的解剖学定位研究

摘　　要：【目的】探讨顶内沟在脑标本的准确定位,为侧脑室三角区肿瘤的顶内沟手术入路提供形态学依据。【方法】选用正常成人头部标本30例,定位并解剖顶内沟,测量顶内沟和中央后沟交点与大脑纵裂、中央沟的距离;测量颞上线与缘上回上缘的最短距离;描述顶内沟在MRI特殊层面上出现的特征。【结果】顶内沟和中央后沟交点与大脑纵裂的距离:左侧(41.50±0.17)mm、右侧(40.74±0.45)mm;顶内沟和中央后沟交点与中央沟的距离:左侧(17.18±0.46)mm、右侧(16.49±0.67)mm;测量颞上线与缘上回上缘的最短距离:左侧(19.34±0.26)mm、右侧(18.79±0.54)mm。【结论】顶内沟和中央后沟的交点与中央沟、大脑纵裂的距离可作为侧脑室三角区肿瘤切除经顶内沟入路判断顶内沟位置的依据;颞上线与缘上回上缘的最短距离可作为手术中识别缘上回的标志。[Objective]To study the anatomical localization of interparietal sulcus and provid the morphological evidence for surgical treatment of tumor in lateral ventricle trigone via interparietal sulcus approach.[Methods]30 normal brain specimens were prepared.The skull was opened and marked the interparietal sulcus,superior temporal line,gyrus supramarginalis.The lengths from the intersection of interparietal sulcus and postcentral sulcus to diastematia and central sulcus were measured.The shortest length from superior temporal line to superior border of gyrus supramarginalis were measured.The present characteristics of the interparietal sulcus on special MRI images were described.[Results]The lengths from the intersection of interparietal sulcus and postcentral sulcus to diastematia and central sulcus were（41.50±0.17）mm/（17.18±0.46）mm in left hemisphere respectively and（40.74±0.45）mm/（16.49±0.67）mm in right hemispherea.The shortest length from superior temporal line to superior border of gyrus supramarginalis is（19.34±0.26）mm in left hemispherea and（18.79±0.54）mm in right hemisphere.[Conclusions]The lengths from the intersection of interparietal sulcus and postcentral sulcus to diastematia and central sulcus can be used as a evidence to judge the location of interparietal sulcus in surgical management of tumor in lateral ventricle trigone via interparietal sulcus approache;The shortest length from superior temporal line to superior border of gyrus supramarginalis can be served as a marker to identify the gyrus supramarginalis in operation.
