摘 要:目的探索计算机辅助诊断对脑梗死进行早期临床分型的可行性。方法回顾性分析200例脑梗死患者的临床资料,其中进展型脑梗死69例,非进展型脑梗死131例,利用逐步判别分析法对脑梗死进行相关分析,并用交互验证法进行验证。结果逐步判别分析筛选出脑梗死类型、入院神经功能缺损评分、梗死部位总数、WBC升高、TIA史、低血钙、入院时高血糖和高密度脂蛋白水平共8个进展型脑梗死的独立预测因素,根据这些因素分别建立进展型和非进展型脑梗死的判别式,交互验证法对脑梗死分型的正确判别率平均为81.5%。结论脑梗死分型的计算机辅助诊断有助于早期发现进展型脑梗死的高危患者。Objective To investigate the feasibility of classifying the early stage ischemic stroke by computer aided diagnosis system.Methods The clinical data of 200 cases(69 progressive and 131 non progressive ischemic stroke patients) were retrospectively analyzed.Differentiate method was performed to analyze the ischemic stroke.All cases were tested by interactive validation.Results Infarction category,high density lipoprotein,TIA history,number of infarction sites,higher WBC,hypocalcaemia,higher blood sugar and lower neurologic impairment score on admission were the independent predictive factors for progressive ischemic stroke according to differentiate analysis.The differentiate formulation for progressive and non progressive ischemic stroke diagnosis were setup.The accuracy rate of classifying ischemic stroke by interactive validation was 81.5%.Conclusion Computer aided diagnosis for classifying ischemic stroke is useful to find out patients with high risk to develop progressive ischemic stroke in early stage.
