摘 要:清除龋坏牙本质是Nd:YAG激光在口腔科的重要应用之一。本研究目的是评价Nd:YAG激光应用于清除龋坏牙本质的效果以及对龋坏下正常牙本质形态改变的作用。实验用Nd:YAG激光照射处理离体牙牙本质浅龋区,然后对实验区作扫描电镜观察。结果发现:激光参数在15pps、60mJ/p时,牙本质表面龋坏组织基本清除干净,牙本质小管口缩小,管间牙本质粗糙。激光参数设置在15pps、80mJ/p时,龋坏组织全部消失,牙本质小管口封闭,牙本质熔融并出现再结晶。当激光参数为15pps、100mJ/p时,表面结构完全丧失,出现高度碳化晶体。本实验得出结论:Nd:YAG激光可以有效地清除龋坏牙本质,并造成牙本质表面形态改变。可以推论:Nd:YAG激光导致的这种牙本质表面结构变化有利于充填体固位,有利于减少充填后敏感症及充填后继发龋的发生。Caries removal is one of the important potential uses of Nd:YAG laser. The puropse of this study is to evaluate the effect and the dentin morphological changes from Nd:YAG laser treatment for removing caries. In the study, dentin caries on extracted teeth were lased and then examined under SEM. It was found at low energy of 60mJ/p and a frequency of 15pps, caries was almost cleared, tubular orifices reduced, peritubular dentin became rough. Energy of 80mJ/p removed all caries leaving a melted and recrystallized surface. When the energy reached 100mJ/p, the ultrastructure of dentin was throughly destroyed. In concluson, Nd:YAG laser is effective on removing caries in dentin and causing superficial morphological changes. These changes could do good to reduce post-restorative sensitivity and prevent secondary caries.
