摘  要:【目的】研究抑癌基因细胞凋亡蛋白酶活化因子-1(Apaf-1)和腺瘤性结肠息肉病相关基因(APC)的启动子甲基化状况与膀胱癌临床病理的关系及对患者复发的影响。【方法】采用甲基化特异性PCR方法检测110例膀胱癌组织中Apaf-1和APC的启动子甲基化状态,以15例正常膀胱黏膜组织为对照组。【结果】Apaf-1和APC基因在膀胱癌组织中的甲基化率为90.0%和82.7%,在正常膀胱组织为6.77%和20.0%,差异分别有统计学意义(P均=0.000)。Apaf-1及APC的甲基化率与肿瘤分化程度及临床分期有明显的相关性,随肿瘤分级、分期的增加而升高。在复发组与未复发组的比较中,Apaf-1的甲基化阳性率分别为97.6%(41/42)和85.3%(58/68),差异有统计学意义(χ2=4.382,P=0.036),而APC的甲基化率分别为85.7%(36/42)和80.9%(55/68),差异无统计学意义(χ2=0.424,P=0.515)。【结论】Apaf-1及APC的甲基化改变在膀胱癌的发生、发展过程中具有重要作用,Apaf-1启动子甲基化可能成为膀胱癌患者预后判断的分子标志物。【Objective】To study the Apaf-1 and APC promoter methylation status and its relation with clinical pathology and prognosis of bladder carcinoma.【Methods】Methylation status of Apaf-1 and APC gene promoter in 110 specimens of bladder carcinoma and 15 normal bladder tissues were detected by methylation-specific PCR.【Results】The methylation rate of Apaf-1 and APC were significantly higher in bladder carcinoma(90.0%,82.7%)than that of normal bladder tissues(6.77%,20.0%) respectively with statistically significant differences(P=0.000).The methylation rate of Apaf-1 and APC correlated well with the different grades and stages of bladder carcinoma respectively,increased with the development of tumor grades and stages.Between the recurrent group and the non-recurrent group,the methylation rates of Apaf-1 were 97.6%(41/42)and 85.3%(58/68),with statistically significant differences(χ2=4.382,P=0.036);APC were 85.7%(36/42)and 80.9%(55/68),with no statistically significant differences(χ2=0.424,P=0.515).【Conclusion】The methylation status of Apaf-1 and APC was related closely to the development of bladder carcinoma,the methylation status of Apaf-1 may become a new potential biomarker of prognosis in bladder carcinoma.
