摘 要:目的探讨白血病化疗中并发肺部侵袭性真菌感染的多层CT影像学表现及诊断价值。方法对100例白血病患者治疗中并发肺部侵袭性真菌感染的CT影像表现进行分析。病例符合确诊标准11例,符合临床诊断89例。结果 CT表观病灶位于双肺89例,右肺8例,左肺3例。病源菌以白假丝酵母菌者最为常见,共51例,热带假丝酵母菌4例,曲霉菌42例,克柔假丝酵母菌1例,无名假丝酵母菌合并烟霉菌1例,曲霉菌合并白假丝酵母菌1例。CT影像学表现将肺部真菌感染分为5型:弥漫型65例,复合型15例,实变型13例,结节或团块型6例,曲菌球型1例。结论多层CT在白血病化疗中并发肺部真菌感染的诊断和临床治疗观察中具有重要参考价值。Objective To explore multi - slice CT findings and value of CT diagnosis of pulmonary invasive fungal infection during chemotherapy in leukemia. Methods 100 CT findings of pulmonary invasive fungal infection during chemotherapy in leukemia were analyzed. 11 cases agreed with the definitive criteria, and 89 cases met the clinical diagnosis. Results 89 cases were found focuses in both sides of the lung, 8 in the right and 3 left. The eandida albicans was the most common pathogenic fungus, found in 51 cases (51%) , eandida tropicalis was found in 4 cases, aspergillus 42 cases, candi- da krusei 1, eandida famate with mold Fungus 1 , and aspergillus with eandida albicans 1. Five different types of pulmonary invasive fungal infection were categorized according to CT findings, diffuse type 65 cases, composite type 15, eonsolidative 13, nodular or massive 6, and aspergilloma 1. Conclusion Multi - slice CT diagnosis of pulmonary invasive fuugal infection during chemotherapy in leukemia is worthwhile in clinical intervention and observation.
