摘  要:目的探讨周围髓鞘蛋白22(peripheral myelin protein 22,PMP22)、髓鞘碱性蛋白(myelin basic pro-tein,MBP)在周围神经活检标本中的表达差异及其诊断意义。方法收集35例成人腓肠神经活检标本,进行抗PMP22、MBP免疫组织化学染色,对其灰度值做定量分析,并与相应的光镜和电镜下组织学形态进行比较分析。结果 PMP22和MBP的表达部位并不完全相同;PMP22表达水平与有髓纤维髓鞘的残存数目、完整程度及无髓纤维-Schwann细胞单位的数量、功能状态等多个因素相关;MBP表达水平主要与残存的有髓纤维的数量和髓鞘脱失的程度相关;两者的表达具有一定的相关性,但PMP22更多反映Schwann细胞状态,而MBP反映有髓纤维髓鞘的状态。结论周围神经活检中PMP22和MBP的检测有助于判断受损髓鞘的神经纤维分布以及Schwann细胞的功能状态,可考虑作为周围神经活检诊断的常规检测。Objective To investigate the expression difference between peripheral myelin protein 22 (PMP22) and myelin basic protein (MBP) in peripheral nerve biopsy specimens and its diagnostic significance. Methods Thirty - five eases of adult sural nerve biopsy specimens were collected to perform anti - PMP22, - MBP immunohistochemieal stain. And their gray values were analyzed quantitatively to compare with corresponding tissue morphologies under light and electron mieroseopies. Results The expressions of PMP22 and MBP were not completely on the same site. PMP22 expression was related to factors such as survival number, integrity of myelinated fibers, number and functional status of non - myelinated fiber - Schwann cell units, while MBP expression mainly to survival number of myelinated fiber and degree of demyelination. There was a certain correlation between 2 expressions, but PMP22 reflected more Sehwann cell state while MBP the state of myelinated fiber demyelination. Conclusion Detection of PMP22 and MBP in peripheral nerve biopsy, helping determine the distributions of damaged myelin nerve fibers and the functional status of Schwann cells, can be routinely used in peripheral nerve biopsy.
