摘　　要：目的：研究表皮生长因子受体（EGFR）和增殖细胞核抗原（PCNA）在胃肿瘤中的表达及其临床意义。方法：应用免疫组化技术（sABC法），对胃肿瘤组织切片（胃腺癌性息肉10例，胃癌94例）中EGFR和PCNA进行半定量检测，观察其与胃肿瘤临床病理参数及预后的关系及其相关性。结果：①EGFR在正常胃粘膜和胃腺瘤性息肉中均无表达，在胃癌中的表达率为40．4％，胃粘膜不同病理状态下PCNA指数差异显著（P＜0．01）；②EGFR和PCNA指数的表达与患者的性别和年龄不相关（P＞0．05），而与肿瘤的生长方式、浆膜层受侵与否、组织类型、淋巴结转移和分期有相关性（P＜0．05）；③EGFR和PCNA表达与患者的生存期密切相关（P＜0．05），EGFR阳性或PCNA强级（3～4级）者预后差；④EGFR和PCNA在胃癌中的表达有相关性（P＜0．05）。结论：EGFR和PCNA检测可作为判断胃癌生物学行为、预测预后和指导治疗的指标。Objective:To study the expression of EGFR and PCNA protein in human gastric neoplasm and its clinical significance.Methods:The expression of EGFR and PCNA was semiquantitatively analyzed immunohistochemically in 10 gastric adenomatous polyp and 94 gastric carcinomas using the strept avidin-biotin-peroxidase complex (sABC) technique.Results:①The expression of EGFR was negative in normal gastric mucosae and gastric adenomatous Polyp while 40. 4% in gastric carcinomas (GC). PCNA labeling index(LI) showed a significant difference in different pathologic state. (P <0. 01); ②The expression of EGFR and PCNA in GC was independent of sex and age (P>0.05), but correlated with the growing matiner of the tumor, serosal infiltration, growth style of tumor, metastasis to perigastric lymph nodes and clinical stages (P<0.05); ③The expression of EGFR and PCNA in GC correlated with the patients' survival rate (P <0. 05),i.e, patients with EGFR positive expression or a high grade of PCNA expression generally had a worse pmgnosis; ④The expression of EGFR correlated positively with PCNA expression in GC (P < 0. 05). Conclusion: These data suggest that EGFR and PCNA LI are fairly good index to indicate biologic behavior, prognosis and treatment in gastric c^inoma.
