成人急性非淋巴细胞白血病M2型患者AML1/ETO融合基因mRNA表达及其临床意义

摘　　要：目的分析成人急性非淋巴细胞性白血病M2型患者AML1/ETO(AE)融合基因的表达,并探讨其与临床表现,疗效的关系以及在预后判断中的意义。方法采用巢式逆转录聚合酶链反应技术分析成人急性非淋巴细胞性白血M2患者AE融合基因mRNA表达,并比较其与临床表现、治疗缓解率的关系。通过随访,评价其在预后判断中的意义。结果在23例成人急性非淋巴细胞性白血M2患者中,12例可检测到AE融合基因mRNA表达,为所检测患者的52.2%(12/23),其中,11例为M2a,1例为M2b。与AE融合基因mRNA表达阴性患者比较,AE融合基因mRNA表达阳性患者年纪较轻,外周血白细胞计数较低,临床上易出现发热,出血。经治疗后,AE融合基因mRNA表达阳性患者缓解率为75.0%,而AE融合基因mRNA表达阴性患者缓解率为36.4%。经8个月随访发现,1例AE融合基因mRNA表达阴性患者在缓解后第4个月复发并出现脑内转移而死亡,另1例在缓解后第7个月复发。而AE融合基因mRNA表达阳性患者在8个月的随访中无一例复发。结论在成人急性非淋巴细胞性白血病M2患者中50%以上表达AE融合基因mRNA,AE融合基因mRNA表达阳性患者更易出现发热,出血倾向,但治疗效果较好,预后良好。Objective To study the expression of AE fusion gene mRNA in ANLL-M2 patients and the relationship between expression of AE and clinical manifestations,efficacy and prognosis.Methods The expression of AE fusion gene mRNA was analyzed by reverse transcription-polymerase chain reaction（RT-PCR）.Based on the result of expression of AE gene,patients were classified into two groups： AE-positive group（A） and AE-negative group（B）.Then,clinical manifestations and efficacy of patients in these two groups were compared.Additionally,its significance in prognosis after follow-up for 8 months was evaluated.Results The expression of AE fusion gene mRNA was found in 12 of 23（52.2%） ANLL-M2 patients including eleven case of M2a and a case of M2b.In comparison with AE-negative patients,AE-positive patients were younger and easier to get fever and bleeding.After induction,75.0%（9 /12） of AE-positive patients achieved complete remission（CR）,only 36.4%（4/11） of AE-negative patients died.After follow up for 8 months,one AE-negative patient relapsed in fourth month after CR and died due to the invasion of central nerve system by leukemia.One AE-negative patient relapsed in seventh month.But none of AE-pos-itive patients relapsed.Conclusion AE fusion gene mRNA is expressed in more than 50% ANLL-M2 patients.AE-positive patients are easier to get fever and bleeding than those in AE-negative patients,but their efficacy of treatment and prognosis are better.
