启东肝癌中黄曲霉毒素白蛋白加成物水平和p53蛋白表达的研究

摘　　要：对３０例启东肝癌病例和１６例对照用竞争抑制放射免疫法检测血清黄曲霉毒素白蛋白加成物，７例血清ＨＢｓＡｇ、抗－ＨＢｃ均阳性的手术病例的石蜡包埋组织用免疫组化法检测ｐ５３蛋白表达。结果表明，病例组黄曲霉毒素白蛋白加成物水平在１．０７～３．４６ｐｍｏｌＡＦＢ１／ｍｇ白蛋白之间，对照组在０．８５～２．９９ｐｍｏｌＡＦＢ１／ｍｇ白蛋白之间，两组比较无显著性差异。６例癌组织、４例癌旁组织ｐ５３蛋白表达阳性，且这６例阳性者黄曲霉毒素白蛋白加成物平均水平为１．８１５±０．６２２ｐｍｏｌＡＦＢ１／ｍｇ白蛋白。提示较长时间内病例和对照黄曲霉毒素暴露的蓄积剂量并无显著性差异。启东肝癌病例ｐ５３蛋白表达率很高。Thirty cases of hepatocellular carcinoma(HCC)diagnosed between March and October,1996 and sixteen controls selected from other non digestive and non tumorous patients in Qidong Liver Cancer Institute were enrolled in our study.Radio immunoassay(RIA)was performed to detect aflatoxin albumin adducts in serum samples.Immunohistochemical methods was used to study the p53 overexpression.The mean±SD of binding of cases was 2 031±0 658pmolAFB 1/mg albumin while which of controls was 1 975±0 573 pmolAFB 1/mg albumin.There was no significant difference between the two groups.In HCC tissue and dajacent non tumorous liver tissue,six and four of seven cases had increased expression of p53 respectivly.The seven cases were all positive for HBsAg and anti HBc.The mean±SD of aflatoxin albumin adducts of these six HCCs was 1 815±0 622 pmolAFB 1/mg albumin.Our data showed that the accumulative dose of aflatoxin food intake during a long period wasn't different between cases and controls.The rate of p53 overexpression was high among Qidong HCCs,but the reasons remained uncertiain.
