摘  要:【目的】探讨奶制品摄入量与代谢综合征(MS)及其危险因素的关系。【方法】在广州市人民街社区采取分层随机抽样法选取459名、年龄40~70岁符合要求的研究对象,用食物频数问卷(FFQ)法评价奶制品及其它膳食摄入情况,测量身高、体质量、腰围和血压,检测空腹血脂和血糖。根据每日奶制品摄入量将研究对象分为3组,即从不摄入组(男66人,女43人)、中等摄入组(男54人,15mL/d;女120人,44mL/d)和较高摄入组(男54人,128mL/d;女122人,219mL/d),参照国际糖尿病联盟(IDF2005)的MS诊断标准,分析MS患病率、危险因素与奶制品摄入的关系。【结果】校正年龄、膳食因素后,奶制品的摄入与腰围负相关(P for trend=0.035),与空腹血糖负相关(P for trend=0.027);奶制品摄入较高的人相比于从不摄入奶制品的人,腰围低4.3%,空腹血糖低5.0%。奶制品的摄入与BMI(P for trend=0.053)、收缩压(P for trend=0.056)、甘油三酯(P for trend=0.065)的负相关性具有临界统计学意义。在logistic回归分析中,奶制品摄入与MS患病风险存在负相关,3组研究对象的OR(95%CI)分别为:从不摄入组:1.0(对照);低摄入组:0.538(0.289-1.000);高摄入组:0.531(0.285-0.986),校正混杂因素后趋势减弱。奶制品摄入与MS患病风险的负相关性在男性中更加显著(P for trend=0.016)。【结论】奶制品摄入与MS及其危险因素之间存在负相关。【Objective】 To evaluate the association of dairy consumption with the components of the metabolic syndrome. 【Methods】 A sample of 459 men and women aged 40-70 years was randomly selected. Dairy consumption and other food groups were assessed by using a food-frequency questionnaire. Height,weight,waist circumference,and blood pressure were measured. A fasting blood sample was collected for lipid and glucose measurements. The metabolic syndrome was defined according to the International Diabetes Federation (IDF) 2005. Data were analyzed according to tertiles of medians of dairy products. 【Results】 Dairy consumption was significantly and negatively associated with waist (P for trend = 0.035) and fasting plasma glucose (P for trend = 0.027) in total subjects after adjusting for age and dietary factors including total energy,meat,egg,vegetables,fruits and soy. Subjects with high dairy consumption had 4.3% and 5.0% lower in waist and fasting plasma glucose than those with never consumption of dairy products. The negative relationships between dairy consumption and BMI,systolic blood pressure and triglycercide were borderline significant (P for trend = 0.053,0.056 and 0.065,respectively). In logistic regression the odds ratio for metabolic syndrome according to dairy consumption were 1.0 in never intake group (reference),0.538 (95% confidence interval,0.289,1.000) in low intake group,0.531 (0.285,0.986) in high intake group. The trend was more significant in male subjects. 【Conclusion】 In an elderly Chinese population,higher dairy consumption was associated with more favorable levels of components of the metabolic syndrome.
