不同时期上海市高中生肥胖及非酒精性脂肪性肝病比较分析

摘　　要：目的探讨上海地区2001年、2009年在校高中生肥胖和非酒精性脂肪肝(NAFLD)检出率,为预防青少年肥胖和NAFLD提供依据。方法分别于2001年和2009年调查上海城区2所高中,共942名在校学生身高、体质量,并进行肝脏超声检查。结果 2001年组483名高中生超重、肥胖、NAFLD检出率分别为13.7%(66/483)、3.9%(19/483)、4.8%(23/483);2009年459名在校高中生超重、肥胖、NAFLD检出率分别为16.3%(75/459)、8.5%(39/459)、7.0%(32/459)。NAFLD在体质量正常组检出率仅为0.3%～0.5%,超重组10.7%～15.2%,肥胖组57.9%～59.0%,三组之间检出率差异有统计学意义(P<0.001)。2001年男生肥胖检出率5.7%(16/281),2009年为14.6%(31/213),2009年男生肥胖检出率显著高于2001年(P<0.001)。结论上海地区在校高中生男生肥胖检出率呈上升趋势,肥胖和超重者的NAFLD危险性明显增加。Objective To investigate the prevalence of obesity and nonalcoholic fatty liver disease（NAFLD） at different period in senior high school students in Shanghai for providing scientific basis for the early prevention of obesity and NAFLD in adolescents.Methods The height and weight measurement and liver ultrasonography were recorded in 942 students came from two senior high schools of Shanghai in 2001 and 2009,respectively.Results The incidences of overweight,obesity,and NAFLD were 13.7%,3.9%,and 4.8% in 483 students in 2001,and 16.4%,8.5%,and 7.0% in 459 students in 2009,respectively.Furthermore,the significant difference in the incidence of NAFLD（P〈0.001） was observed,and consisting of 0.3% ～ 0.5% in the group with normal body mass index（BMI）,10.7% ～ 15.2% in the overweight group,and 57.9% ～ 59.0% in the obesity group,respectively.There were more male students with obesity in 2009 compared to those in 2001（14.6% vs 5.7%,P〈0.001）.Conclusions The prevalence of obesity in senior high school students tends to increase in Shanghai.The occurrence of NAFLD is more frequent in adolescences with overweight or obesity.
