染色体遗传不稳定性在宫颈癌病因中的研究

摘　　要：目的分析宫颈癌组织3号染色体的杂合性丢失(LOH)、微卫星不稳定性(MI)及其与临床病理参数之间的关系,探讨遗传不稳定性在宫颈癌发生发展中的作用。方法选择D3S1234、D3S1300、D3S1481三个微卫星位点,应用PCR-聚丙烯酰胺-银染技术对67例宫颈癌组织进行杂合性缺失和微卫星不稳定性分析。结果 67例宫颈癌中有48例(71.6%)至少有一个位点LOH阳性,有25例(37.1%)至少有一个位点MI阳性;LOH阳性率Ⅲ、Ⅳ期大于Ⅰ、Ⅱ期,鳞癌明显高于腺癌,差异均有统计学意义(P<0.05),在高、中、低分化癌中差异无统计学意义(P>0.05);MI阳性率Ⅲ、Ⅳ期大于Ⅰ、Ⅱ期,腺癌高于鳞癌,差异有统计学意义(P<0.05),低分化癌高于中、高分化癌,差异有统计学意义(P<0.05)。MI阳性者均为LOH阳性。结论 3号染色体短臂可能存在宫颈癌相关抑癌基因,LOH及MI是宫颈癌发生发展中的重要事件。Objective To analyze the loss of heterozygosity（LOH） and microsatellite instability（MSI） and their relationship to each other as well as to clinic pathological factors at partial sites of chromosome in human cervical carcinoma,and to investigate the roles of genetic instability in the development of diseases.Methods Three microsatellites sites were selected.LOH and MSI were detected by polymerase chain reaction（PCR） and denaturing polyacrylamide gel electrophoresis in 67 cases of cervical carcinoma.Results In 67 cervical carcinoma cases,48 cases（71.6%） had LOH positive at least on 1 site,25（37.1%） had MI positive on at least 1 site.LOH positive rate was higher in stages Ⅲ,IV than in stages I,Ⅱ,and in squamous cell carcinoma than in adenocarcinoma（P〈 0.05）,but no significant difference was noted between well-,moderately-and poorly-differentiated cancers（P 〉0.05）.MI positive rate was higher in stages Ⅲ,IV than in stages I,Ⅱ,in adenocarcinoma than in squamous cell carcinoma（P〈 0.05）,and in poorly-differentiated cancers than in moderately-and well-differentiated cancers（P 〈0.05）.All MI positive cases were LOH positive.Conclusion Suppressor gene related to cervical cancer may exist in short arm of chromosome 3,and LOH and MI are important events in development of cervical caners.
