摘 要:目的:深入研究超声测定胎儿颈项皮肤厚度(NFT)的方法及其临床应用价值。方法:(1)研究4种成像角度及不同超声频率对NFT测值的影响;(2)建立正常胎儿NFT参考值;(3)研究NFT增厚的临床价值。结果:(1)胎头自然俯屈(成像角度145°~155°)或胎头稍仰伸(成像角度135°~145°)NFT差异无统计学意义(P>0.05);(2)15~38周两种频率测量孕周相同胎儿NFT差异有统计学意义(P<0.05);(3)NFT增厚的胎儿中,结局不良者占60.9%。结论:在胎儿颈项正中矢状切面,胎头自然俯屈或胎头稍仰伸两个角度均可作为测量NFT值的标准切面;应用高频超声测量更精确;中期妊娠测定NFT能明显提高异常胎儿检出率,适用于广大基层医院。Objective:To study the method for ultrasonic measurement the fetal nuchal fold thickness(NFT) and its value of clinical application.Methods:(1)The influence of four imaging angles and ultrasonic frequency for NFT were compared.(2)The reference standard of normal NFT was established.(3)Clinical value of thickened NFT was studied.Results:(1)When imaging angle was between 135°~155°,there was no significant difference(P0.05).(2) For NFT of 15~38 week normal fetus,there was significant difference between 5-2MHz and 12-5MHz measurement(P0.05).(3)In thickened NFT,bad outcome was 60.9%.Conclusions:In median sagittal section of fetal occipital and two angles with normal flexion or slight extension of fetal head can be the standard plane to measure the NFT.Fetal NFT with high frequency ultrasonography can be accurate.Measurement the NFT in second trimester can obviously enhance the detection rate of chromosomal abnormalities in prenatal diagnosis.
