营养状况主观综合评估在炎症性肠病患者营养评价中的作用

摘　　要：目的研究营养状况主观综合评估（subjective global assessment,SGA）在炎症性肠病（inflammatory boweld isease,IBD）患者营养评价中的作用。方法收集2008年9月~2010年2月在我院就诊符合IBD诊断标准的85例,采用SGA方法评估其营养状况,同时测定血红蛋白、白蛋白、前白蛋白、白细胞、总淋巴细胞计数（TLC）、血钾、钠、钙水平。结果本组SGA分级营养良好（A级）占21.18%,轻、中度营养不良（B级）占44.71%,重度营养不良（C级）占34.12%。重度营养不良患者血红蛋白水平明显低于营养良好者,轻、中、重度营养不良患者血清白蛋白水平低于营养良好患者,差异均有统计学意义（P〈0.05）。不同营养状况分级患者年龄、TLC、血清前白蛋白浓度、白细胞计数、血钙、钾、钠浓度比较差异无统计学意义（P〉0.05）。结论 SGA方法作为定性评估蛋白质-能量营养状况方案,简便易行,可以为临床医生的综合快速地判断IBD营养不良提供一定依据。Objective To investigate the application of subjective global assessment（SGA） in nutritional evaluation on patients with inflammatory bowel disease（IBD）.Methods 85 IBD patients who underwent examination during September 2008 and February 2010 were enrolled.Nutritional status was assessed by SGA.Serum hemoglobin,albumin,pre-albumin,WBC,total lymphocyte count（TLC）,Ca2＋,K＋ and Na＋ were analyzed.Results The SGA process classified 21.18% as well nourished group,44.71% as mildly malnourished group and 34.12% as severely malnourished group.Serum hemoglobin in severely malnourished group was obviously lower than that of the well nourished group,albumin in mildly malnourished group and severely malnourished group was obviously lower than that of the well nourished group,and the difference was statistically significant（P0.05）.There was no significant difference of age,TLC,serum prealbumin concentration,WBC and blood Ca2＋,K＋ and Na＋ level in different nutritional status group（P0.05）.Conclusion SAG as a simple qualitative assessment of protein-energy nutritional status of the program can help physicians to assess the nutritional status of IBD patients quickly and effectively.
