摘　　要：目的 探讨血红蛋白琼脂糖凝胶片电泳用于地中海贫血筛查的价值.方法 选取124例疑似地贫（小细胞低色素或红细胞脆性〈63%）的标本,进行血红蛋白琼脂糖凝胶片电泳,对α地中海贫血表型阳性[HbA2〈2.5%或出现异常血红蛋白快带（HbH、HbBart＇s）]或者β地中海贫血表型阳性（HbA2〉3.5%或HbF〉3.2%）的标本,进行地中海贫血基因检测.结果 124例中α地中海贫血表型阳性60例,占48.4%,基因检测确诊51例,符合率85%,Χ2检验二者无显著性差异（P〉0.05）;β地中海贫血表型阳性36例,占29.0%,基因检测确诊30例,符合率83.3%,Χ2检验二者无显著性差异（P〉0.05）.结论 血红蛋白琼脂糖凝胶片电泳用于地中海贫血筛查有较高的敏感性和特异性.Objective To evaluate the effect of hemoglobin agarose gel eleetcophoresis in screening for thalassemia. Methods 124 patients with suspected thalassemia were selected （microcytic hypochromic anemia or the erythrocyte osmotic fragility 〈 63% ）, The hemoglobin were analyzed by agarose gel eleetrophoresis, genetic testing for thalassemia samples on the α - thalassemia phenotype positive [ HbA2 〈 2.5 % , or faster with abnormal hemoglobin （ HbH, HbBart＇ s） ] or β - thalassemia phenotype positive （ HbA2 〉 3.5 % or HbF 〉 3.2 % ）. Results 60 patients positive for α - thalassemia phenotype of 124 eases, accounting for 48.4 %, genetic testing confirmed 51 cases, the coincidence rate of 85%, there was no significant difference between them by X2 test （P 〈 0.05） ; β - thalassemia phenotype positive for 36 eases, accounting for 29.0%, genetic testing confirmed 30 cases, the coincidence rate of 83.3%, there was no significant difference between them by X2 test （P 〈 0.05）. Conclusions Hemoglobin agarose gel eleetrophoresis in screening for thalassemia have a higher sensitivity and specificity.
