摘  要:目的探讨P16和survivin在人脑胶质瘤组织中的表达及临床意义。方法按Kernohan四级分类法将96例胶质瘤分为Ⅰ级26例、Ⅱ级32例、Ⅲ级28例和Ⅳ级10例,另外12例正常脑组织作为对照组,采用免疫组化S-P法检测P16和sur-vivin的表达情况。结果在正常对照组和胶质瘤组中P16、survivin阳性表达率分别为91.7%、0%和53.1%、58.3%,两者比较差异均具有统计学意义(P<0.05)。Ⅰ、Ⅱ、Ⅲ、Ⅳ级脑胶质瘤中P16蛋白的阳性表达率分别为80.8%、59.4%、32.1%、20.0%,而survivin的阳性表达率分别为38.4%、53.1%、75.0%和80.0%,各组间差异均具有统计学意义(P<0.05),Ⅲ、Ⅳ级脑胶质瘤的P16阳性表达率明显低于Ⅰ、Ⅱ级而survivin的阳性表达率明显高于Ⅰ、Ⅱ级(P<0.05)。P16蛋白表达与survivin蛋白表达呈负相关(r=-0.415,P<0.05)。结论 P16蛋白的表达缺失和survivin蛋白的过度表达与脑胶质瘤恶性程度相关,它们在其发生、发展中可能起协同作用。Objective Explore P16 and survivin in the brain tissue of gliomas expression and clinical significance. Methods According to Kernohan level 4 classification,96 patients with glioma were divided into Ⅰ level 26 cases, Ⅱlevel 32 cases, Ⅲlevel 28 cases and Ⅳ level 10 cases,and 12 cases of normal brain tissue as control group,using immunohistochemistry S - P method to detect P16 and survivin expression. Resultsfn normal controls and glioma group,P16 .survivin positive expression rates were 91.7%, 0% and 53.1%,58.3% ,both comparative difference was statistically significant (P〈0.05). Ⅰ,Ⅱ , Ⅲ , Ⅳlevel glioma P16 protein in the positive expression rates respectively were 80.8 %, 59.4 %, 32.1 %and 20.0 %, but survivin positive expression rates respectively are 38.4%,53.1%,75.0 % and 80.0% ,both comparative difference was statistically significant (P〈0.05), Ⅲ, Ⅳ level of gliomas P16 positive expression rates were obviously lower than that of Ⅰ and Ⅱ , but survivin positive expression rate was obviously higher than that of I and Ⅱ (P〈0.05). P16 protein expression was negatively correlated with survivin protein (r= -0. 415 ,P〈0. 05). Conclusion P16 protein expression flaw and excessive survivin protein expression is closely related to malignant glioma degree, they may play the occurrence and development of synergistic action.
