摘 要:目的使用PentacamHR三维眼前段分析系统量化年龄相关性白内障超声乳化吸出联合人工晶状体植入术手术前后的角膜容积、前房深度及角膜像差的改变。方法年龄相关性白内障54例(54眼)进行超声乳化吸出联合人工晶状体植入术。术前和术后3个月使用PentaeamHR眼前段分析系统测量的前房深度、房角宽度、角膜容积和角膜像差等数值进行数据分析,差异有统计学意义(P〈0.05)。结果术后3个月,前房深度为术前的1.40倍或1.34倍,前房角增宽为术前的1.17倍,差异均有统计学意义(P〈0.05)。在角膜3mm、5mm、7mm和10mm范围之内的角膜容积分别增加了2.79%、3.21%、3.42%和3.70%。角膜后表面的像差(Z2-2、Z20、Z3-1、Z40、Z5-1、Z51和Z60)较术前有明显增加,差异有统计学意义(P〈0.05)。结论PentaeamHR三维眼前段分析系统不但可以量化超声乳化吸出联合人工晶状体植入术后的角膜容积、前房深度和角膜像差的变化,而且较以往的检查仪器具有非接触、检查时间短、性能高等特点,为我们评估手术对术眼结构的影响、提高患者视觉质量提供了良好的基础。Objective To evaluate the influences of phacoemulsification with intraocular lens im plantation(IOL) on corneal volume and aberrations obtained with the newly developed high resolution(HR) Pentacam Scheimpflug camera in patients with age related catract. Methods In a prospective study, we e valuated 54 eyes of 54 patients having cataract phaeoemulsification and intraocular lens implantation in Zhongshan Ophthalmic Center. We performed Pentacam HR measurements preoperatively and at 3 months af ter surgery. A paired t test was used to compare preoperative and 3 months postoperative measurements of an terior chamber depth (ACD) , anterior chamber angle (ACA) width, corneal volume (CV) and all the anteri or,posterior and total aberrations (Cornea). The postoperative values were compared against the respective preoperative vallues with SPSS 13.0. Results The differences between ACD, ACA and CV values taken preoperatively and those taken postoperatively at 3 months were statistically significang(P 〈0.05 ). The ante- rior chamber was 1.40 times deeper and the anterior chamber angle was 1.17 times wider at 3 months after surgery than that preoperatively. The CV increased by 2.79% ,3.21% ,3/42% ,3/70% on the 3 mm,5 mm, 7 mm, 10 mm circle area respectively, at 3 months after surgery. Posterior corneal surface aberrations parame tern(Z2-2 ,Z20,Z3-1 ,Z40,Z5-1 ,Z51 and Z60) increased significantly compared with the preoperative and the differences were statistically significant ( P 〈 0.05 ). Conclusions The newly developed PentaeamHR could be used to evaluate the change of CV, ACD and aberrations with non contact, short time and high- performance.
