摘  要:目的 研究卵巢肿瘤组织中nm23H2 基因的扩增、表达及其突变并探讨其临床意义。 方法 用免疫组织化学(IHC) 、反转录多聚酶链反应(RTPCR)和多聚酶链反应单链构象多态技术(PCRSSCP) 方法检测41 例卵巢癌、20 例卵巢良性肿瘤和21 例正常卵巢组织nm23H2 基因的表达、扩增和突变。 结果 ①卵巢癌nm23H2 阳性表达率在68 % 以上,显著地高于良性肿瘤及正常卵巢组织(P< 001) ;②癌灶nm23H2 的阳性表达及扩增显著地高于转移灶( P< 001) ;③Ⅰ~Ⅱ期患者癌组织中nm23H2 的阳性表达率显著地高于Ⅲ~Ⅳ期者( P<001) ;④nm23H2 的阳性表达和扩增与病理分级及病理分类无显著差异( P> 005) ;⑤nm23H2 阳性表达的患者近期疗效比阴性者好( P< 005)。 结论 nm23H2 阳性表达是卵巢恶性肿瘤的早发事件,其阳性表达的降低预示疗效差。因此,对患者进行nm23H2 基因的检测有助于预测预后。To study the amplification,expression and mutation of nm23 H2 gene and their clinical significance in ovarian cancer.METHODS Immunohistochemistry,reverse transcription polymerase chain reaction (RT PCR) and single strand conformation polymorphisms (PCR SSCP) procedure were used to detect the mRNA, protein and mutation of nm23 H2 gene expression in 41 ovarian cancers,20 benign tumors and 21 normal controls.RESULTS ①The positive rate greater than 68% of nm23 H2 gene expression in ovarian cancer was significantly higher than that in benign tumors and normal controls( P <0 01) ②The positive expression rate of nm23 H2 gene in primary site was significantly higher than that in adjacent sites and metastatic lesions( P <0 01).③The positive expression rate of nm23 H2 gene in Ⅰ~Ⅱ stages of patients was significantly higher than that in Ⅲ~Ⅳ stages of patients( P <0 01) ④No significant correlation was observed between the positive expression rate of nm23 H2 gene and pathological type or grade in ovarian cancer( P >0 05) ⑤The short term curative effect in patients with positive expression of nm23 H2 gene was better than that with low expression( P <0 05).CONCLUSIONS The positive expression of nm23 H2 gene exists in earlier stages of ovarian cancer.The patients with low expression of nm23 gene have a poorer curative effect.Thus,the detection of nm23 gene expression in tumor patients may be of value clinically in predicting prognosis.
