血清瘦素、TNFα、HA、PCⅢ在酒精性脂肪肝及肝炎中表达的意义

摘　　要：目的探讨血清瘦素、TNFα及HA、PCⅢ等指标在酒精性肝病患者的表达情况,为临床早期干预提供依据。方法用放射免疫分析法测定20例正常成人和49例酒精性肝病患者的血清瘦素、TNFα及HA、PCⅢ水平。结果酒精性脂肪肝(AFL)组、酒精性肝炎(HA)组、正常对照(NC)组血清瘦素、TNFα及HA、PCⅢ水平分别为(5.79±2.96)μg/L、(0.95±0.53)μg/L、(127.73±32.37)μg/L、(116.23±21.97)μg/L,(9.94±4.67)μg/L、(1.14±0.21)μg/L、(171.45±72.23)μg/L、(142.42±63.14)μg/L,(3.86±2.87)μg/L、(0.69±0.12)μg/L、(118.49±28.94)μg/L、(107.37±22.81)μg/L,AFL组血清瘦素、TNFα水平与NC组比较有显著性差异(P<0.05);AH组清瘦素、TNFα、HA和PCⅢ水平与NC组比较有显著性差异(P<0.05、P<0.01);AFL组血清瘦素、TNFα、HA水平与AH组比较亦有显著差异(P<0.05、P<0.01)。结论早期检测酒精性肝病患者血清瘦素、TNFα水平,对延缓肝纤维化进展有益。Objective To study expression of Leptin,TNFα,liver fibrosis criteria in alcoholic fatty liver and alcoholic hepatitis,and evaluate the predictive value of them in alcoholic liver disease.Methods In 49 case of alcoholic liver disease and 20 normal cases,the expression of Leptin,TNFα,liver fibrosis criteria were examined by using radioimmunoassay method.Results 1.The expression of Leptin、TNFα in alcoholic fatty liver cases were significantly higher than those in normal cases（P〈0.05）.2.The expression of Leptin,TNFα,liver fibrosis criteria in alcoholic hepatitis cases were significantly higher than those in normal cases（P〈0.05,P〈0.01）.3 The expression of Leptin,TNFα,liver fibrosis criteria（HA） in Alcoholic Hepatitis cases were significantly higher than those in Alcoholic fatty liver cases（P〈0.05,P〈0.01）.Conclusions Detection of Leptin,TNFαexpression in alcoholic liver disease might be useful in assessing the development,judging prognosis and selecting clinical therapy.
