多模态融合图像与病理学在盆腔肿瘤靶区勾画中的差异性研究

摘  要:目的探讨CT、MRI及其融合图像的盆腔肿瘤勾画靶区(GTV)最大径与病理肿瘤实体的差异性。方法 (1)选取6例边界相对完整的术前盆腔肿瘤患者分别进行CT、MRI异机非同步扫描。CT和MRI图像数据行手工配准后传送至飞利浦公司PINNACLE V8.0放射治疗计划系统,行图像融合并由一位有经验的放疗科医师对患者的CT、MRI及融合图像肿瘤靶区(GTVs)进行勾画,并测量其最大径;(2)术后立即对实体肿瘤行速冻处理,并对实体肿瘤由外到里2mm一层行病理切片直到在镜下找到肿瘤细胞,测量镜下肿瘤的最大径并与(1)勾画的肿瘤靶区最大径比较分析。结果 DFUSION相对D实体平均缩小了0.205cm,约为D实体的2.80%、DCT和DMRI相对D实体平均缩小了0.548和0.458cm,约为D实体的7.48%和6.25%。结论通过融合图像勾画的GTV更接近于实体肿瘤,与单独CT或MRI图像相比优势明显。CT/MR图像融合技术有利于盆腔肿瘤靶区的确定,提高了临床对盆腔肿瘤靶区(GTV)勾画的准确率,利于患者的诊治。【Objective】 To evaluate the difference between the images of CT,MRI and their fusing images of GTV and pathologic tumor maximum diameter.【Methods】(1)6 patients who suffered from pelvic cancer with relatively complete border underwent CT and MRI before surgery.Image of CT and MRI were delivered to Pinnacle3 V8.0(Philip cor.) radiation treatment planning system to make image fusion after manual registration.GTVs were delineated and tumor maximum diameters were measured by an experienced radio therapist.(2)Quick freeze the entity tumor after surgery and specimens were then serially sectioned at 2mm intervals till tumor cells were detected.Measured the maximum diameter of the tumor with microscopy and compared the maximum diameters measured with CT,MRI and their fusion images.【Results】 Comparing to pathologic examination,DFUSION reduced about 0.205cm in average,which was approximately 2.80% of it.DCT and DMRI,reduced 0.548cm and 0.458cm respectively,which were 7.48% and 6.25% of pathology.【Conclusion】 Comparing to CT or MRI separately,GTV with fusion images are obviously closer to the tumor entity.Image fusion technology profits in contouring target and improved the accuracy of GTV,which have great contributions in diagnosis and therapy of cancer.
