摘  要:目的:探讨磷脂酰肌醇蛋白聚糖3(GPC3)mRNA和甲胎蛋白(AFP)mRNA在肝细胞癌(HCC)患者外周血中联合检测对HCC诊断价值。方法:采用荧光定量RT-PCR技术检测72例HCC患者、22例肝良性病变组和20例健康组外周血中GPC3 mRNA和AFP mRNA表达。结果:HCC患者GPC3 mRNA和AFP mRNA表达显著高于肝良性病变组和健康组(P<0.05);肝良性病变组和健康组中表达无统计学意义(P>0.05)。两者表达与肿瘤大小、是否侵袭和病理分级有关(P<0.05);AFP mRNA表达还与血清AFP浓度有关(P<0.05);年龄、性别、有无HbsAg感染和包膜与两者表达无关(P>0.05)。单项检测灵敏度、准确度较低;联合检测灵敏度达90.3%,准确度达92.1%;两者在HCC中表达未存在直接关联(P>0.05)。结论:联合检测GPC3 mRNA和AFP mRNA的表达有助于HCC的早期临床诊断。Objective:To explore the value of glypican-3(GPC3)mRNA and alpha-fetoprotein(AFP)mRNA in peripheral blood in diagnosis of hepatocellular carcinoma(HCC).Methods: Fluorescent quantitative reverse transcription polymerase chain reaction was employed to detect the expression of GPC3 mRNA and AFP mRNA in peripheral blood from patients with HCC(n=72),nonHCC tumor samples(n=22)and controls(n=20).Results: The levels of GPC3 mRNA and AFP mRNA were significantly higher in HCC than nonHCC tumor samples and controls(P〈0.05),the levels were not much different from those in nonHCC tumor samples and controls(P〈0.05).The levels of GPC3 mRNA and AFP mRNA seemed to be correlated with tumor diameter,the stage(by TNM classification)of HCC and/or distant metastasis(P〈0.05),the levels of AFP mRNA also seemed to be correlated with the serum AFP value(P〈0.05).With combined detection of GPC3 mRNA and AFP mRNA,the sensitivity would be 90.3%,accuracy would be 92.1%.Moreover,there wasn′t correlation between the levels of GPC3 mRNA and AFP mRNA.Conclusion: Detected the expression of GPC3 mRNA and AFP mRNA is useful for diagnosis of primary HCC.
