托吡酯单药治疗对特发性癫患儿免疫功能的影响

摘　　要：目的观察托吡酯单药治疗对特发性癫患儿免疫功能的影响。方法选取新诊断特发性癫患儿39例（其中男23例,女16例;年龄5个月~11岁）定期门诊随访,用药前、用药3个月、6个月分别采集空腹静脉血以全自动生化分析仪采用免疫比浊法检测血清免疫球蛋白IgG、IgA、IgM及流式细胞分析仪检测CD3、CD4、CD8及细胞间黏附分子（ICAM）-1水平。结果癫患儿用药前血清IgA、IgG、ICAM-1、CD8水平升高,CD3、CD4下降,与健康对照组比较差异有统计学意义;而血清IgM与健康对照组比较差异无统计学意义。服药6个月癫患儿血清IgA、IgG、CD8、ICAM-1水平降低,与服药前比较差异有统计学意义,与健康对照组比较差异无统计学意义;而血清IgM、CD3、CD4水平升高与用药前及与健康对照组比较差异均无统计学意义。结论特发性癫患儿用药前即存在免疫功能紊乱,托吡酯可改善癫患儿的免疫功能。Objective To explore the influence of topiramate monotherapy on immune function in children with idiopathic epilepsy.Methods Thirty-nine children with idiopathic epilepsy（23 boys and 16 girls,aged from 5 months to 11 years） with adopting follow-up of out-patients before and after treatment in 3 and 6 months and venous blood was collected respectively from the children on empty stomach.Applying automatic biochemical analyzer to detect serum immunoglobulin lgG,IgA,IgM through immune turbidimetry;while CD3,CD4,CD8 and intercellular adhesion molecule-1（ICAM-1）were detected through the application of flow cytometry.Results Compared with the healthy control group,IgA,IgG,ICAM-1,CD8 levels increased and CD3,CD4 levels decreased in the children with epilepsy before treatment and there were distinctive differences,whereas IgM had no difference,compared with that of healthy control group.After treatment for 6 months,IgA,IgG,ICAM-1,CD8 levels decreased,there were distinctive differences compared with that before treatment,while there was no difference compared with that of healthy control group.IgM,CD3,CD4 levels increase had no statistic difference,compared with both that in healthy control group and that in the epilepsy group before treatment.Conclusions There are already immune function disorders in the children with idiopathic epilepsy before taking antiepileptic drugs;topiramate monotherapy can improve the immune function of children with epilepsy.
