颅内侧支循环的建立与脑动脉闭塞后的相关性分析

摘　　要：目的:研究缺血性脑卒中患者侧支循环病变部位及其建立的数量与脑动脉闭塞后的关系。方法:回顾性分析30例住院的脑梗死患者,经头部MRI检查、DSA检查发现至少有1支以上脑动脉闭塞。利用SPSS13.0软件对所得数据进行卡方检验,从而探讨侧支循环建立的数量与病变部位数量之间的关系。结果:我们发现单侧支循环代偿25例,其中责任血管区单部位脑梗死9例,多部位脑梗死16例;多侧支循环代偿25例,其中责任血管区单部位脑梗死16例,多部位脑梗死9例,共有51支血管闭塞(多支血管闭塞18例),其中前循环闭塞血管27支,后循环闭塞血管24支。结论:脑动脉闭塞后侧枝循环建立的程度与完整的Willis环及有效侧枝循环的数量有关,研究表明,多支侧支循环建立,可显著减少梗死数目。Objective： To study the relationship between the lesion regions of collateral circulation,its established numbers and cerebral arterial occlusions in ischemic stroke cases.Methods： 30 ischemic stroke cases were retrospectively analyzed.They were performed MRI in the head and DSA,the findings showed that every patient had at least one cerebral artery occlusion.The relationship between the quantity of collateral circulation and quantity of ischemic stroke location were explored by SPSS 11.5 statistics software.Results： Of the 30cases,51 arterial occlusions were found by chance.DSA demonstrated internal carotid artery occlusion occurred in 27 cases,vertebral artery,basilar artery occlusion in 24cases.DSA study showed that all these cases had at least one collateral circulation.25 cases of single collateral circulation had 9 cases of single ischemic stroke locations and 16 cases of multiple ischemic stroke locations;25 cases of multiple collateral circulation had 16 cases of single ischemic stroke locations and 9 cases of multiple ischemic stroke locations.Conclusion： Effective collateral circulation can be established spontaneously through multiple ways when cerebral artery occlusion takes place.If intact Wills circle exists.It is suggested multiple collateral circulation can significantly decrease ischemic stroke location quantity in this study.
