摘  要:目的探讨牛乳制品对非甾体消炎药所致小肠黏膜损伤的形态学结构及黏膜中表皮生长因子(EGF)表达的影响。方法雄性SD大鼠80只,随机分为5组(每组16只),空白对照组和模型组予自由饮水,其余3组从第1天起分别予10%牛乳、10%牛初乳、酸牛乳管饲,第5天时除空白对照组外其余各组予双氯芬酸15mg/kg管饲1次。于造模后24h和48h分别评价小肠大体损伤评分、病理损伤评分,检测绒毛高度、肠黏膜超微结构变化。免疫组化法观察小肠黏膜EGF的表达。结果24h和48h时点牛初乳组大体和病理损伤评分均低于模型组。牛初乳组24h和48h小肠绒毛高度分别为(145.7±16.5)μm和(139.2±19.0)μm,明显高于模型组[(119.2±19.2)μm和(105.4±18.4)μm],P〈0.05。牛初乳组透射电镜下见小肠黏膜微绒毛较整齐,扫描电镜下见小肠绒毛表面光滑,顶端微绒毛密集;而模型组、牛乳组、酸牛乳组的超微结构改变相似,透射电镜下可见微绒毛水肿,排列紊乱,部分脱落,扫描电镜下见小肠黏膜表面粗糙不平,局部破溃,微绒毛减少甚至缺失。牛初乳组48h时点小肠黏膜EGF阳性面积为(6170.5±1483.9)μm2,显著高于模型组[(3517.1±985.8)μm2],P〈0.05。牛乳组、酸牛乳组的大体损伤评分、病理损伤评分、小肠绒毛高度、EGF阳性面积与模型组相似,差异无统计学意义。结论牛初乳可有效防治双氯芬酸所致小肠黏膜损伤,维护小肠黏膜机械屏障的完整,推测其机制可能与牛初乳中的成分可维持小肠黏膜内EGF含量有关。Objective To investigate the effect of milk and milk products on morphological structure and epidermal growth factor (EGF) of non-steroidal anti-inflammatory drugs (NSAIDs) induced small intestinal damage in animals. Methods Eighty male SD rats were randomly divided into 5 groups: control group, diclofenac group, diclofenac with 10% low fat milk group , diclofenac with 10% colostrum group and diclofenac with yoghurt group. The animals with milk or colostrum or yoghurt were fed for 5 days before the administration of diclofenac with 15 mg/kg by gavage, once. Then they were observed the scores of anatomical lesion and the scores of tissue damage of mucous membrane and the height of villous at the 24th and 48th hour after making the models. Observation of the change of uhrastructural organization of mucous membrane was carried out with transmission and scanning electron microscope and immunohistochemistry of EGF. Results The scores of anatomical lesion and tissue damage of mucous membrane of the colostrum group were lower than those of the diclofenac group ( P 〈 0. 05 ). The heights of the pile on small intestine of the 24th and 48th hour of the eolostrum group were (145.7 ± 16. 5 ) μm and (139. 2 ± 19. 0 ) μm, respectively. They were higher than those of the diclofenac group [ ( 119. 2 ±19. 2) μm and ( 105.4 ± 18. 4) μm, P 〈 0. 05 ]. However there was no difference of the scores and the height among diclofenac group, milkgroup and yoghurt group. TEM and SEM of tissues showed that the cytoplasmic membrane and other cellular components of villous epithelial cells were well preserved in colostrum group, and the microvilli in the milk group and yoghurt group were ablated more obviously. The positive area of EGF of small intestine [ (6170. 5 ± 1483.9)μm2 ] were higher 48 h after administration of diclofenac compared with the diclofenac group (P 〈 0. 05 ).
