焦虑抑郁障碍共病患者的特质焦虑与其血清BDNF水平的相关性研究

摘　　要：目的探讨焦虑抑郁障碍共病患者的特质焦虑与其血清人脑源性神经营养因子(brain-derived neurotrophic factor,BDNF)变化水平的关系,分析特质焦虑是否对共病患者治疗效果产生影响。方法收集重医附一院门诊及住院部焦虑抑郁障碍共病患者64例,以及正常对照组人群60例,随机给予患者抗抑郁药帕罗西汀或舍曲林进行治疗,分别于治疗前,治疗后第2、4、8、12周使用焦虑状态-特质量表(state-trait anxiety inventory,STAI)中T-AI量表部分对患者特质焦虑程度进行测评,同时ELISA法测定患者及正常人群相应的血清BDNF水平。结果①治疗前焦虑抑郁障碍共病患者血清BDNF水平低于正常人群(P<0.05),共病患者T-AI量表评分高于正常人群(P<0.05)。②治疗后T-AI量表评分异常者较T-AI量表评分正常者血清BDNF水平恢复速度更慢。③患者的特质焦虑与其血清BDNF水平呈负相关(r=-0.502,P<0.05)。结论焦虑抑郁障碍共病患者的特质焦虑与其血清BDNF水平相关,药物作用起效时间相对更晚。Objective To study the correlation between trait anxiety and serum brain-derived neurotrophic factor（BDNF）level in patients with combined anxiety and depression and analyze whether trait anxiety affects the therapeutic effect in such patients.Methods Sixty-four patients with combined anxiety and depression and 60 normal controls were enrolled in this study.Patients with combined anxiety and depression were treated with paroxetine or sertraline and their trait anxiety was assessed with the trait anxiety inventory（T-AI） scale in the state-trait anxiety inventory（S-TAI）before and 2,4,8,12 weeks after treatment.Serum BDNF levels in patients with combined anxiety and depression and normal controls were measured by ELISA and analyzed using the SPSS18.0.Results The serum BDNF levels were lower and the trait anxiety scores were higher in patients with combined anxiety and depression than in normal controls before treatment（P0.05）.The recovery time of serum BDNF levels was longer in patients with combined anxiety and depression who had abnormal T-AI scores than in those who had normal T-AI scores（P0.05）.The trait anxiety in patients with combined depression and anxiety was negatively correlated with their serum BDNF levels（r=-0.502,P0.05）.Conclusion Trait anxiety in patients with combined depression and anxiety is negatively correlated with their serum BDNF levels and the action of drugs in them is relatively later.
