摘　　要：目的观察四步疗法治疗新生JLN囊炎的临床疗效。方法对1000例（1216眼）新生JLN囊炎根据不同年龄、发育特点逐步采用四部治疗方法。第1步：年龄〈3个月的患儿采用按摩方法；第2步：按摩无效且年龄≥3个月-6个月患儿行加压泪道冲洗，每月1次；第3步：加压泪道冲洗失败且年龄〉6个月的患儿行泪道探通术，每月1次；第4步：2次探通失败者，行KTP激光泪道成形术。随访6～12个月。结果按摩治愈率为25．47％，泪道加压冲洗治愈率为70．19％，泪道探通术治愈率为81．93％，KTP激光泪道成形术治愈率为98．97％。所有患儿均未出现假道及泪点撕裂。结论采用四部疗法循序渐进治疗新生儿泪囊炎，损伤小，疗效快，方便，患儿家属易接受。Objective To observe the clinical effect of 4-step therapy for neonatal dacryocystitis. Methods we treated 1000 case（ 1216 eyes） of neonatal dacryocystitis with 4-step therapy methods according to their age and growth characteristic. The first step： the infants less than 3 months old had undergone massage. The second step ：positive pressure lacrimal passage irrigation was done for one time every month if the former method failed or the infant older than 3 months. The third step： probing was done for one time every month if the second step failed or the infant older than 6 months. The forth step ： KTP laser therapy was done after two probing failed. The following-up time was 6-12 months. Results The cure rate of massage was 25.47% , that of lacrimal passage irrigation was 70.19% , that of probing was 81.93 % , that of KTP laser therapy was 98.97%. There was no false passage formation or lacimal point laceration. Conclusion Progressive 4-step therapy for neonatal dacryocystitis is convenient, safe and its effect is fast. It is easy to be accepted by kids and relatives.
