囊袋张力环在马凡综合征晶状体半脱位手术中的应用

摘　　要：目的探讨囊袋张力环在马凡综合征晶状体半脱位手术治疗中的应用效果。方法对32例(54眼)马凡综合征伴晶状体半脱位患者在晶状体摘出联合人工晶状体植入术中,将囊袋张力环植入晶状体囊袋后再植入后房型人工晶状体。术后定期随访,观察术后患者视力、疗效和并发症等。结果 32例54眼患者均手术成功,术后患者视力均有不同程度的提高;术后3个月最佳矫正视力>0.6者15眼,0.3～0.6者36眼,<0.3者3眼。随访期间未发现人工晶状体脱位、眼压升高及玻璃体脱出等并发症发生。结论囊袋张力环应用于马凡综合征晶状体半脱位摘出手术可以提高手术安全性,防止人工晶状体偏位,避免或减少玻璃体脱出,是马凡综合征伴晶状体半脱位患者行超声乳化白内障手术的有效辅助手段。Objective To evaluate the efficacy of capsular tension ring（CTR） during the operation of Marfan＇s syndrome with lens subluxation.Methods About 32 cases（54 eyes） of Marfan＇s syndrome with lens subluxation were implanted with capsular tension ring and posterior chamber intraocular lens（IOL） after lens extraction.Regular follow-up was taken postoperatively.The treatment effects and visual acuity were recorded.Results The capsular tension rings and posterior chamber intraocular lens were implanted successfully in all the 32 cases（54 eyes）.All postoperative visual acuities were improved in various degrees.At postoperative 3 months,there were 15 eyes with best corrected visual acuity more than 0.6,36 eyes between 0.3 to 0.6,3 eyes less than 0.3.No serious complication such as IOL dislocation,ocular hypertension or vitreous prolapse was seen in all cases.Conclusions The capsular tension ring is safe to the patients of Marfan＇s syndrome with lens subluxation.It can prevent the decentration of intraocular lens,avoid or reduce the vitreous overflowing and other complications in patients with Marfan＇s syndrome.It is an auxiliary means for Marfan＇s syndrome with lens subluxation.
