中、低位直肠癌新辅助放疗30例临床分析

摘　　要：目的探讨中、低位直肠癌术前短期三维适形放射治疗的效果。方法选择本院2006年6月至2008年8月收治的局部进展期中低位直肠癌患者64例。经CT或磁共振成像(MRI)评估可切除后,随机分为放疗组和对照组,治疗组30例,在术前短期放疗后4～6周进行手术;对照组34例,接受单纯接受手术治疗。结果放疗组和对照组的保肛率分别为86.6%和70.6%,二者比较差异有统计学意义(P<0.05);放疗组与对照组3年局部复发率分别为6.6%和17.6%,二者比较差异有统计学意义(P<0.05)。结论术前放疗不仅可提高直肠癌手术保肛率,同时降低局部复发率,是一种较好的综合治疗措施。Objective To investigate the clinical efficacy of preoperative short-term conformal radiotherapy for middle and lower rectal cancer.Methods The clinical data of 64 cases of middle and lower rectal cancer admitted to Chongqing tumour hospital from June 2006 to August 2008 were randomly divided into two groups,a treatment and a control group.In the treatment group,30 patients received preoperative short-term radiotherapy followed by surgery 4~6 weeks later,while 34 patients in control group just received surgery.Results The rates of anal preservation of the control and treatment group were 70.6% and 86.6% respectively,there was significant difference between the two groups（P0.05）.The 3 years local recurrence rates of the two groups were 6.6% and 17.6%,there was significant difference between the two groups（P0.05）.Conclusion Preoperative radiotherapy for middle and lower rectal cancer can improve the rate of anal preservation and decrease the local recurrence rate,so it is a better multimodality treatment technique and should be widely applied for middle and lower rectal cancer.
