简易营养评估法在中国老年肺癌手术患者中的应用

摘　　要：目的通过将修订后的MNA量表及MNA-SF量表应用于中国老年肺癌手术患者人群中,了解中国老年肺癌手术患者的营养状况并进行初步评价。方法利用修订后的MNA量表对103例新入院的老年肺癌需手术的病人进行营养评估,同时收集病人的基本情况、人体测量指标、生化指标、术后并发症等相关资料。结果根据MNA修订界值后的评分标准,所调查103例(男:n=66;女:n=37)住院老年肺癌手术患者营养不良的患病率为12.6%(13/103),潜在营养不良为31.1%(32/103),营养正常为56.3%(58/103);MNA平均得分(23.6±3.7);MNA-SF平均得分(11.5±2.4),MNA与BMI、上臂肌围、腓肠肌围和Hb存在正相关(P<0.05);MNA-SF与年龄存在负相关(P<0.05),与BMI、上臂肌围、腓肠肌围和MNA总分之间存在正相关(P<0.05)。结论修订界值后的MNA量表适合对中国老年肺癌手术病人的营养状况进行合理有效的评估,评估结果表明老年肺癌手术病人的营养不良发生率较高。Objective Through the application of modified MNA and MNA-SF scale in older Chinese operative inpatients with lung cancer,their nutrition status was evaluated primarily and got general information. Methods One hundred and three older Chinese inpatients with lung cancer,who would have an operation,were enrolled in this study.Collected information included preoperative evaluation in modified MNA and MNA-SF,general information of patients,anthropometric measurements and biochemical markers.Results According to the modified cutoff point of the full MNA in the 103 patients（male：n=66;female：n=37）,12.6% of those assessed were malnourished,31.1% were at risk of malnutrition and 56.3% were well nourished.MNA total scores averaged 23.6±3.7.Positive correlations were found between MNA,MNA-SF,body mass index,arm muscle circumference,calf circumference and hemoglobin（P0.05）.Negative correlation were found between MNA-SF and age（P0.05）.Conclusions Modified MNA scale is fit to evaluate the nutrition status of older Chinese inpatients with lung cancer.Evaluation shows that there is high prevalence of malnutrition among this population.
