摘  要:目的探讨PTEN蛋白、MAPK信号及周期素Cyclin D1表达与肺癌预后的相关性。方法应用免疫组织化学染色法,检测PTEN蛋白、MAPK信号及周期素Cyclin D1在肺癌组织中的表达情况,采用Cox回归模型及Kaplan-Meier进行单因素分析。结果肺癌中PTEN蛋白阳性率为75%,其阳性表达与肿瘤大小、淋巴结转移及患者吸烟情况显著相关(P<0.01),PTEN阳性表达与非小细胞肺癌分化程度显著相关。肺癌中MAPK蛋白阳性率为33.87%,其阳性表达与肿瘤淋巴结转移及分化程度显著相关(P<0.05)。肺癌中Cyclin D1蛋白阳性率为66.13%,其阳性表达率与患者吸烟及肿瘤淋巴结转移显著相关。PTEN与MAPK蛋白表达呈明显负相关(P<0.05)。采用Cox比例风险模型进行多因素分析,结果提示吸烟、肿瘤分化程度、PTEN蛋白表达及临床分期4个变量是患者生存危险因素。采用Kaplan-Meier生存曲线与Wilcoxon检验结果示:PTEN表达阴性患者较阳性患者生存时间短。结论 PTEN、MAPK、Cyclin D1蛋白在肺癌中呈阳性表达是一个常见事件,PTEN蛋白表达与肺癌预后密切相关。Objective To explorethe correlation of the expression of PTEN protein,MAPK signal andcell cycle regulation factorCyclinD1 with the prognosis of lung cancer.Methods Using immunohistochemical staining method to detect the expression of PTEN protein,MAPK signal and Cyclin D1 in lung cancer tissue.The results were analyzed single-factored under Cox-Model and Kaplan-Meier assay.Results The positive rate of expression of PTEN protein in lung cancer was 75%,which was significantly correlated with the size of tumor,thelymph node metastasis and the smoking behavior of the patients(P0.01).The expression level of PTEN was significantly correlated with the differentiation degree of NSCLC.The positive rate of expression of MAPK protein in lung cancer was 33.87%,which was significantly correlated with the tumor lymph node metastasis and the differentiation degree of NSCLC(P0.05).The positive rate of expression of Cyclin D1 in lung cancer was 66.13%,which was significantly correlated with the smoking behavior and the lymph node metastasis.It is apparently that the expression of PTEN protein and MAPK protein was in the negative correlation(P0.05).Analysis under the Cox-Model leads to the results which indicate that the four variables smoking behavior,tumor differentiation degree,expression of PTEN protein and TNM stage were the risk factors of the survival span of patients.Analysis under Kaplan-Meier assay and Wilcoxon method showed that patient with a negative PTEN expression got a shorter survival span compared to whom with a positive one.Conclusion The positive expression of PTEN,MAPK and Cyclin D1 protein was a frequent event in lung cancer.The expression of PTEN was significantly correlated with lung cancer prognosis.
