摘 要:目的使用剑桥神经心理自动化成套测试(CANTAB)探讨青少年品行障碍与执行功能的关系。方法采用韦氏成人智力测验(WAIS)简易版、CANTAB中的内外空间成套变换(IED)、剑桥袋球(SOC)和空间工作记忆(SWM)对86名品行障碍青少年和110名职高学生进行对照研究。结果品行障碍青少年的智商水平(98.17±5.07)低于对照组[(103.42±5.40),P<0.01];在IED中,品行障碍青少年的外维转换前期错误数、外维转换期错误数、错误总数[(12.23±9.15)、(16.69±10.57)、(35.44±11.09)次]均多于对照组[(8.31±3.80)、(13.07±10.31)、(25.63±12.24)次,P<0.01],阶段完成数[(7.47±1.96)次]少于对照组[(8.29±0.91)次,P<0.05];在SOC中,品行障碍青少年完成任务的最少步数(7.65±1.96)多于对照组[(6.70±2.05),P<0.01],3步和5步的平均步数多于对照组(P<0.05),4步和5步的起始思考时间短于对照组(P<0.05);在SWM中,品行障碍青少年总的组间搜索错误数[(27.56±18.88)次]多于对照组[(20.47±18.40)次,P<0.01],在6个和8个盒子中的组间搜索错误数均多于对照组(P<0.01),策略分[(33.84±4.58)分]高于对照组[(32.00±5.05)分,P<0.01]。结论品行障碍青少年在执行功能方面存在一定的缺陷。Objective To study the correlation between conduct disorder and executive function of adolescents using Cambridge neuropsychological test automated battery (CANTAB). Methods A control study was performed in 86 adolescents with conduct disorder and 118 normal controls using Wechsler adult intel- ligence scale short form (WAIS-S), intra/extra dimensional set shift (IED), stockings of Cambridge (SoC) and spatial working memory (SWM) in CANTAB. Results The intelligence quotient (IQ) of adolescents with conduct disorder was lower than that of normal controls (98.17 ± 5.07 vs 103.42 ± 5.40, P 〈 0.01 ). IED showed that the number of pre-ED errors, EDS errors, and total errors was greater in adolescents with conduct disorder than in normal controls ( 12.23 ± 9.15, 16.69 ± 10.57 and 35.44±11.09 vs 8.31 ± 3.80, 13.07 ± 10.31 and 25.63 ± 12.24, P 〈0.01 ) while the number of stage-solved problems was less in adolescents with conduct disorder than in normal controls (7.47 ± 1.96 vs 8.29 ± 0.91, P 〈 0.05). SOC demonstrated that the minimum moves to solve problems were significantly more in adolescents with conduct disorder than in normal controls (7.65 -+ 1.96 vs 6.70 _+2.05, P 〈0.01 ). The mean moves (3 and 5) to solve problems were signifi- cantly more while the starting time to consider the 4 and 5 moves was shorter in adolescents with conduct disor- der than in normal controls (P 〈 0.05 ). SWM displayed that the number of errors found in 6 and 8 boxes was greater and the strategy score was higher in adolescents with conduct disorder than in normal controls (27.56 ± 18.88 vs 20.47 ±18.40, 33.84 ±4.58 vs 32.00 ±5.05, P 〈0.01). Conclusion Adolescents with conduct disorder have certain defects in executive function.
