摘 要:目的:探讨下咽鳞癌中STAT3和Cyclin D1的表达及与下咽癌临床病理因素的关系。方法:免疫组化方法检测41例下咽鳞癌中STAT3和Cyclin D1表达,分析STAT3和Cyclin D1与临床病理及预后的关系。结果:STAT3和Cyclin D1的表达阳性率分别为31.7%和41.5%,两者表达水平无显著相关性(P>0.05)。颈部淋巴结转移和化疗不敏感患者中STAT3和Cyclin D1表达阳性率显著高于无颈部淋巴结转移和化疗敏感患者(均P<0.05)。在不同年龄、性别、病理分化程度和T分期患者中,STAT3和Cyclin D1表达均无显著性差异(P>0.05)。STAT3和Cyclin D1表达阳性患者5年生存率分别为12.2%和20.1%,均显著低于表达阴性患者的54.6%和49.3%(P<0.05)。多因素分析显示,STAT3和Cyclin D1表达是下咽鳞癌预后的独立预测因素(P<0.05)。结论:STAT3和Cyclin D1表达与下咽鳞癌患者转移、化疗敏感性和预后有关,可以作为潜在的下咽鳞癌患者转移、化疗敏感性和预后的评估因子。Objective: To investigate the expression of signal transducer and activator of transcription 3(STAT3) and Cyclin D1 in hypopharyngeal squamous cell carcinoma(HPSCC) and their relationships with clinical pathological parameters and prognosis.Methods: Immunohistochemical methods were used to detect the expressions of STAT3 and Cyclin D1 in HPSCC tissues,and the results were analyzed according to the clinicopathological findings or survival data.Results: The expression positive rates of STAT3 and Cyclin D1 in HPSCC were 31.7% and 41.5% respectively,without correlation with each other(P0.05).The positive rate of STAT3 or Cyclin D1 in patients with lymph node metastasis and chemotherapy resistance was significantly higher than in those without lymph node metastasis or in chemotherapy sensitive patients(P0.05,respectively).No significant associations of positive expression rate of STAT3 or Cyclin D1 with gender,age,pathological grade and T stage were observed.Five-year survival rate in patients with negative STAT3 and Cyclin D1 respectively(54.6% and 49.3%,respectively) was higher than that in positive ones(12.2% and 20.1%,P0.05,respectively).Expression of STAT3 and Cyclin D1 were significant indicators of prognosis for HPSCC on multivariate analysis(P0.05,respectively).Conclusion: Expression of STAT3 and Cyclin D1 were related to lymph node metastasis,chemotherapy sensitive and prognosis in patient with HPSCC,and may be served as potential predictive factors for lymph node metastasis,chemotherapy sensitivity and prognosis in patients with HPSCC.
