摘 要:目的研究非小细胞肺癌患者(NSCLC)呼出气冷凝液(EBC)中p16基因突变的临床意义。方法收集30例NSCLC患者和20例体检健康者的EBC标本,提取EBC中的DNA,对β-actin基因扩增阳性的EBC标本进行p16基因1、2、3号外显子PCR扩增,并进行DNA基因测序,用DNASTAR软件进行突变比对,结果进行统计学分析。结果①30例肺癌患者的EBC中,有26例β-actin基因片段扩增阳性,26例中有9例检出p16基因突变,突变率为34.6%。②9例肺癌患者发生p16基因突变的外显子,1号外显子3例2,号外显子5例3,号外显子1例。③26例肺癌患者EBC中β-actin基因片段扩增阳性中,Ⅰ期12例,p16基因突变3例,突变率25%;Ⅱ期7例,p16基因突变2例,突变率28.6%;Ⅲ期7例,p16基因突变4例,突变率57.1%。鳞癌14例,p16基因突变6例,突变率42.9%;腺癌11例,p16基因突变3例,突变率27.3%。结论 NSCLC患者EBC中可以检测到p16基因突变,特异性高,EBC中p16基因检测为肺癌研究提供新方法。Objective Study of patients with non small cell lung cancer (NSCLC) in exhaled breath condensate (EBC) of the clinical significance of p16 gene mutations. Methods 30 patients with NSCLC and 20 healthy physical examination of EBC specimens, extraction of DNA in EBC, the beta actin gene amplification positive EBC specimens were p16 gene exon 1,2 and 3 of PCR amplification, and DNA gene sequencing,using DNASTAR software for mutations in alignment. The results were statistically analyzed. Results ①In 30 patients with NSCLC, the 26 cases of EBC group EBC beta actin gene fragment were amplified positive. In 26 cases ,9 cases were pl 6 gene mutation, the mutation rate of p16 gene was 34. 6%. ②In 9 patients with NSCLC, p16 gene mutation in EBC were exon 1 (3 cases), exon 2 (5 cases), and exon 3 ( 1 case). ③In 26 patients with NSCLC,the rate of p16 gene mutations of patients in Ⅰ stage was 25% (3/12). The rate of p16 gene mutations of patients in Ⅱ stage was 28.6% ( 2/7 ). The rate of p16 gene mutations of patients in Ⅲ stage was 57. 1% (4/7). The rate of p 16 gene mutations of patients in Squamous cell carcinoma was 42.9 % (6/14). The rate of p 16 gene mutations of patients in adenocarcinoma was 27.3 % (3/11 ). conclusions: Patients with non small cell lung cancer ( NSCLC ) in exhaled breath condensate ( EBC ) could be detected by p16 gene mutation, high specificity, p16 gene detection for lung cancer research to provide a new method in EBC.
