面神经受压对微血管减压术治疗面肌痉挛的影响

摘　　要：目的探讨微血管减压术治疗面肌痉挛的术后疗效与神经受压迫程度有无关联及其可能原因。方法上海交通大学附属新华医院神经外科在2009-2010年间经微血管减压术治疗了1 074例面肌痉挛患者,从中找出了48例(A组)面神经受压严重或有明显变性,萎缩的典型面肌痉挛患者,并与同期680例(B组)面神经受压不明显的典型面肌痉挛患者的临床特征、影响学检查,术中所见及术后近期疗效进行回顾性对比分析。结果 A组患者中,痉挛完全缓解37例(77.1%),明显缓解减轻9例(18.8%),好转2例(4.2%),无效0例,总有效率为95.8%。B组患者中,术后症状完全缓解563例(82.8%),明显减轻86例(12.6%),好转29例(4.3%)无效2例(0.3%),总有效率95.4%。两组总有效率差异无统计学意义(P>0.05)。结论微血管减压术治疗面肌痉挛疗效与神经受压程度并无明显关系。Objective To explore the possible correlations between compressive degree of facial nerve and the effect of microvascular decompression（MVD） treatment in patients with hemifacial spasm（HFS）.Methods A total of 1 047 patients with HFS underwent MVD in department of neurosurgery in Xinhua Hospital during 2009-2010 were studied retrospectively,in which 48 cases（group A） were found with facial nerve seriously compressed or degeneration.Retrospectively comparative analysis was conducted on group A and 680（group B）typical HFS patients whose facial nerve compression was not obvious and had no nerve degeneration.Contents compared included clinical characteristics,three-dimensional time of flight magnetic resonance angiography（3D-TOF MRA）,surgery videos and postoperative curative effect.Results Among patients in group A,77.1% cases were cured（excellent）,18.8% obviously relieved（good）,and the failure and recurrence rates were 0.0%（n = 0） and 2.1%（n = 1）.The total effective rate was 95.8%.Among patients in group B,82.8% cases were cured（excellent）,12.6% obviously relieved（good）,and the failure rate was 0.3%（n = 2）.The total effective rate was 95.4%.The differences between two groups were not statistically significant（P0.05）.Conclusion There was no apparently correlations between compressive degree of facial nerve and the effect of MVD in patients with HFS.
