彩色血管能量成像与cyclin E在甲状腺癌中的表达及其预后的相关性研究

摘　　要：目的研究甲状腺癌中细胞周期素E(cyclin E)的表达与彩色血管能量成像(CPA)的相关性关系,并探讨其临床预后意义。方法术前应用CPA技术检测50例甲状腺癌患者及30例甲状腺腺瘤患者的血流信号,及其阻力指数(RI),术后采用免疫组化法检测该组织中cyclin E的表达。结果 50例甲状腺癌cyclin E的阳性表达率(64%)显著高于甲状腺腺瘤组(33.3%,P<0.05),cyclin E在组织学Ⅲ级的阳性表达率(86.7%)显著高于Ⅰ、Ⅱ级(50%,P<0.05),伴有淋巴结转移的甲状腺癌cyclin E的阳性表达率为83.3%,无淋巴结转移的cyclin E的阳性表达率为40%,两者比较有统计学意义(P<0.05)。cyclin E的表达强度与CPA血流分级呈正相关(r=0.771,P<0.05),甲状腺癌的RI值为0.73±0.05,显著高于甲状腺腺瘤组的RI值(0.55±0.06,P<0.05)。cyclin E的表达强度与RI值呈正相关(r=0.857,P<0.05)。同时cyclin E的表达强度与甲状腺癌患者的生存时间呈负相关(r=0.874,P<0.05)。结论 cyclin E的表达强度与CPA的血流分级及甲状腺癌预后相关,cyclin E的表达是甲状腺癌有效的预后预测因子,为临床早期诊断甲状腺癌和评估预后提供新的理论依据。Objective To study the relationship between color power anglo imaging （CPA） and expression of cyclin E in thyroid tumor, and assess its significance with clinical prognosis. Methods The blood signal and resistance index （RI） of fifty patients with thyroid carcinoma and thirty cases with adenomas were measured preoperatively by CPA whereas the expression cyclin E were examined with immunohistochemistry after operation. Results The positive expression rate （64%） of eyclin E in the group of thyroid carcinoma was higher than that in adenomas group （33.3%, P〈0. 05）. For thyroid carcinoma in clinical stage III, the positive expression rate of cyclin E （86.7%） was significantly higher than that in the stages of I -lI （50% ,P〈0.05）. The positive rate of cyclin E expression in the group of thyroid carcinoma with simultaneous lymph node metastasis was 83.3%. However, such an expression was only 40% in the group without lymph node metastasis. The comparison between these two groups revealed a significant statistics importance. The expression intensity of cyclin E in thyroid carcinoma group correlated positively to the CPA degrees （r=0. 771 ,P〈0.05） and the RI value of malignant group （0.73±0.05） was higher significantly than that of the adenomas group （0.55±0.06 ,P〈0.05）. In addition, a positive correlation for the intensity of cyclin E and RI values （r=0. 857 ,P^O. 05） and a negative correlation for cyclin E intensity and survival time of patients with thy- roid tumor （r=--0. 874,P〈0.05） were also observed. Conclusions The results demonstrated that the expression intensity of cyclin E correlated positively to the degrees of CPA and negatively to the survival time. The integration of cyclin E and CPA may evaluate the patients＇ conditions before and after operation, and hence provide important information for the earlier diagnosis and prognosis judgment.
