摘  要:目的观察左卡尼汀对高渗盐水诱导的小鼠干眼模型角膜和结膜上皮损害的保护作用。方法实验研究。雌性BALB/c小鼠60只,随机分为高渗盐水滴眼组(HO组),高渗联合等渗盐水滴眼对照组(HO+10组),高渗盐水联合1%左卡尼汀滴眼组(HO+1%LCA组),每组20只。对HO组采用500mOsmol/L氯化钠溶液滴眼,每天5次;对HO+10对照组,先滴308mOsmoL/L氯化钠溶液,30min后,再滴500mOsmol/L氯化钠溶液,每天各滴眼5次;对HO+1%LCA组,先滴1%左膏尼汀滴眼液,30min后,再滴500mOsmol/L氯化钠溶液,每天各滴眼5次。持续滴眼28d。分别在第0、7、14、28天进行角膜荧光素染色评分、角膜上皮厚度测量、结膜杯状细胞计数等。另外,在第28天对角膜上皮进行扫描电镜观察和采用冰点法测定BALB/c小鼠的泪液渗透压。采用Kruskal—WallisH检验比较角膜荧光素染色评分,采用完全随机设计资料的方差分析和LSD—t比较角膜上皮厚度、杯状细胞数量、泪液渗透压值。结果HO组、HO+10组较HO+1%LCA组j“现荧光素染色评分显著丌高、角膜上皮厚度显著增加、结膜杯状细胞计数显著下降:第7天起,3个组之间角膜上皮厚度[3个组分别为(27.7±1.1)、(26.5±1.4)、(25.1±1.0)p.m(F=17.701,82.924,125.507;P〈0.01)]、结膜杯状细胞计数[3个组分别为(11.9±1.1)、(12.1±0.9)、(13.3±0.9)(F=9.963,12.548,7.491;P〈0.01)]差异有统计学意义;第14天起,3个之间角膜荧光素染色评分分别为(3.2±0.9)、(2.9±0.7)、(1.7±0.8)分,差异有统计学意义(X2=11.465,P〈0.01)。在第28天,HO组[(327.37±4.20)mOsmol/Ll、HO+IO组I(324.36±5.72)mOsmol/L]较HO+1%LCA组[(308.29±5.72)mOsmol/L]泪液渗透压值显著升高(F=18.970,P〈0.01),角膜上皮表�Objective To evaluate protective effects of L-carnitine on corneal and conjunctival epithelium of mouse dry eye model induced by hyperosmolar saline. Methods Sixty female BALB/c mice at the age of 6-8 weeks were randomly divided into three groups (20 in each ) : Hyperosmolar saline group (HO) , Hyperosmolar and Isosmotic saline group ( HO + IO ) , as well as Hyperosmolar saline and 1% L-earnitinegroup ( HO + 1% LCA). The HO group was treated with 500 mOsmol/L sodium chloride solution; the HO + IO group with 308 mOsmol/L sodium chloride solution first and 500 mOsmoL/L sodium chloride solution 30 minutes later; and the HO + 1% LCA group with 1% L-carnitine eye drop first and 500 mOsmo]/L sodium chloride solution 30 minutes later. Alternately, 5 times a day for 28 days. Corneal fiuorescein staining, corneal epithelial hematoxylin-eosin staining and thickness measurement, conjunctival epithelial periodic acid-schiff reagent staining and goblet cell counting were conducted on 0, 7th, 14th and 28th days, respectively. On 28th day, corneal surface was inspected by scaning electron microscopy and tear osmolarity was measured. Results In HO or HO + IO group, compared with HO + 1% LCA group, the number of goblet ceils was reduced, the score of corneal fluorescein staining and corneal epithelial thickness was elevated : on 7th day, there was difference noticed in the corneal epithelial thickness [ ( 27. 7 + 1.1 ) , (26. 5 ± 1.4), (25. 1 ± 1.0) μm, P 〈0. 01 ] and the number of goblet cell [ ( 11.9 + 1.1 ), ( 12. 1 ±0. 9), (13.3± 0. 9) , P 〈 0. 01 1 in 3 groups; on 14th day, there was evident change in the corneal flurescein staining area[ (3.2 ±0. 9), (2. 9 ±0. 7), ( 1.7 ±0. 8), (X^2 = 11. 465 ,P 〈0. 01 ] in 3 groups. On 28th day, Increased tear osmolarity and lowered microvilli on cornea were examined in HO [ ( 327.37 ± 4. 20 ) mOsmol/ L] or HO + IO [ ( 324. 36± 5. 72 ) mOsmol/L] group compared with HO + 1% LCA [ (
