补充深海鱼油对类风湿关节炎患者C反应蛋白的影响研究

摘　　要：目的探讨类风湿关节炎(RA)患者补充深海鱼油胶囊后的临床表现、C反应蛋白(CRP)等指标的变化。方法将确诊为RA的60例患者随机分为大豆油组和深海鱼油组,分别给予大豆油胶囊及深海鱼油胶囊[含二十碳五烯酸(EPA)1.8 g/d+二十二碳六烯酸(DHA)1.2 g/d],干预12周;干预期间对所有研究对象进行膳食教育。观察干预前后两组患者的临床表现及CRP等,并计算直观模拟标度尺疼痛评分(VAS)、疾病活动度评分(DAS28)和健康评估问卷(HAQ)评分。结果干预后深海鱼油组患者VAS评分增加值及DAS28评分、降低值与大豆油组比较,差异有统计学意义(P<0.05);n-6/n-3多不饱和脂肪酸(PUFAs)比例为(1.70±0.07)∶1组和(3.00±0.21)∶1组患者血清CRP水平均显著低于(5.54±0.11)∶1组(P<0.05)。结论深海鱼油干预RA患者12周,可改善RA患者病情;摄入n-6/n-3 PUFAs比例<5.54∶1比>5.54∶1更有利于降低RA患者的炎性因子CRP水平。Objective To study the changes of clinical manifestations and C -reactive protein （CRP） in rheumatoid arthritis （RA） patients after the treatment of fish oil capsules. Methods 60 patients diagnosed with RA were randomly divided into soybean oil group and fish oil group, with soybean oil group receiving soybean oil capsule and fish oil group receiving fish oil capsule [ eontain!ng EPA （ 1.8 g/d） ＋ DHA （ 1.2 g/d） ] for 12 weeks, during which time all patients were given dietary edu- cation. Clinical manifestations and CRP were observed before and after treatment. Visual analogue scale （VAS）, disease activity scores （DAS） and health assessment questionnaire （HAQ） were all calculated. Results After intervention, VAS and DAS28 of fish oil group showed statistically significant differences compared with soybean oil group （ P 〈 0. 05 ） . CRP in the [ n - 6/n - 3 PUFA= （5.54±0.11：1）] was significantly higher than CRP in the [n-6/n-3 PUFA= （1.70±0.07） ：1] and [n- 6/n - 3 PUFA = （ 3.00±0. 21 ） ： 1, P 〈 0.05 ] . Conclusion 12 weeks intervention against RA by fish oil can improve RA patients＇ condition, n -6/n -3 PUFA 〈5.54：1 is better than 〉5.54：1 for patients＇ stable CRP.
