髓室压力对粘结剂粘结牙本质剪切强度影响的研究

摘　　要：目的评价髓室压力对粘结剂粘结牙本质剪切强度的影响,比较相同髓室压力对不同粘结剂粘结牙本质剪切强度影响的差异。方法 72颗无龋离体第三磨牙随机分为6组(n=12),实验组用15cm水柱模拟髓室压力,对照组髓腔无压力。分别用3种粘结剂(Single-Bond 2、Clearfil SE-Bond、Clearfil S3-Bond)做树脂光固化后,进行剪切强度测试,扫描电镜观察牙本质粘结界面的特性。结果 3种粘结剂在模拟髓室压力情况下粘结时,其剪切强度均下降,差异均有统计学意义;Single-Bond2(13.2±3.5 MPa)和Clearfil SE-Bond(13.7±2.8 MPa)的剪切强度差异无统计学意义(P>0.05),但均显著高于Clearfil S3-Bond(10.4±2.7 MPa)(P<0.05)。3种粘结剂在髓室无压力情况下粘结时,Single-Bond2(22.7±3.3 MPa)明显高于Clearfil SE-Bond(18.2±4.9 MPa)和Clearfil S3-Bond(14.5±3.5 MPa)(P<0.05)。扫描电镜可见实验组树脂突长度和数量均少于对照组。结论髓室压力会对粘结剂粘结牙本质的剪切强度造成影响。不同粘结剂所受影响的程度有所差异,在临床实践中应考虑这一因素。Objective To evaluate the effects of simulated pulpal pressure on the shear bond strength of different adhesives to dentin.Methods Senventy-two extracted human third molars were randomly divided into 6 groups（n=12）.The experimental groups were tested under simulated pulpal pressure of 15cm H2O,and the control groups treated without pressure.Three adhesives（Single-Bond2,Clearfil SE-Bond and Clearfil S3-Bond）were used respectively,and the properties of bond interface between adhesives and dentin were observed with SEM.Results Under simulated pulpal pressure,mean shear bond strength of three experimental groups were lower than those of control groups（P0.05）.The difference between Single-Bond 2 and Clearfil SE-Bond was not significant（P0.05）,while the shear bond strength of Clearfil S3-Bond was lower than that of the other two adhesives（P0.05）.Under simulated pulpal pressure of 0cm H2O,the shear bond strength of Single-Bond 2 was higher than that of the other two adhesives（P0.05）.Compared with control groups,the length and amount of resin tag in the experimental groups were shorter and less.Conclusion Pulpal pressure can affect the shear strength of adhesives and the influence varies among different adhesives.
