摘　　要：目的探讨ABCA1基因R219K多态与颈动脉粥样硬化伴动脉粥样硬化性脑梗死的相关性。方法采用PCR-RFLP技术,以病例对照方式研究182例颈动脉粥样硬化伴动脉粥样硬化性脑梗死患者及229例正常对照者,对ABCA1基因R219K多态进行检测,比较不同基因型与颈动脉粥样硬化伴动脉粥样硬化性脑梗死的相关性。结果 ABCA1基因R219K基因型和等位基因频率在病例组与对照组比较差异有统计学意义(P<0.01)。对年龄、性别、血脂、高血压病、糖尿病、吸烟等危险因素进行校正后行Lo-gistic回归分析,发现在病例组中KK基因型者患病的风险显著降低,为RR基因型的0.34(OR:0.34,95%CI:0.18～0.64)。与RR基因型比较,KK基因型者的血浆三酰甘油(TG)水平较低(P<0.05)。结论 219KK在中国汉族颈动脉粥样硬化伴动脉粥样硬化性脑梗死人群中具有的一定的保护作用。Objective To investigate the association of the R219K polymorphism in ATP-brnding cassette transporter 1（ABCA1） gene with carotid artery atherosclerosis and atherosccrootic cerebral infarction.Methods ABCA1 gene R219K polymorphism in 182 patients with carotid artery atherosclerosis and atherosccrotic cerebral infarction and 229 controls were detected by PCR-RFLP techniques.Different genotypes with individual susceptibility to carotid atherosclerosis and athersccrootic cerebral infarction were compared.Results There was a significant difference of R219K genotype frequencies and allele frequencies in patient group compared with the control group（P0.01）.Logistic regression analysis showed that KK genotype had the lower risk than the VV genotype（OR：0.34,95% CI：0.18 to 0.64）.Subjects carrying KK had the lower TG level than the RR genotype（P0.05）.Conclusion The results confirm that R219K was a protective factor against carotid artery atherosclerosis and atherosccrootic cerebral infarction.
