我院全静脉营养制剂处方的合理性及药师与医师沟通效果分析

摘　　要：目的:了解我院全静脉营养制剂(TPN)处方存在的问题以及药师与医师对此沟通的效果,以促进我院TPN的合理使用。方法:根据拟定的标准,包括处方中有脂溶性维生素但未加脂肪乳,氨基酸、脂肪乳、葡萄糖任何一项缺乏,电解质浓度不合理等,对2010年1月-2011年3月期间我院调配的TPN处方进行审查,并对问题处方进行分类统计,同时分析药师与医师对此沟通的效果。结果:可能存在问题的TPN处方共84例,出现比例最高的问题为有脂溶性维生素但未加脂肪乳(34.52%)、无氨基酸(19.05%)、Ca2+浓度偏高(10.71%)。药师提出问题与医师沟通后,62例处方的医师接受药师意见并修改处方,占73.81%。医师对于药物配伍方面的问题容易接受药师的意见,而对于电解质浓度,葡萄糖、脂肪乳、氨基酸是否加入及其比例的问题存在争议。结论:目前我院TPN处方尚存在一些问题,通过药师与医师的沟通可有效提高TPN处方的合理性。OBJECTIVE：To explore the problems existing in total parenteral nutrition formulations,and the effect of pharmacist-physician communications in order to promote the rational use of TPN in our hospital.METHODS：According to the settled standards,i.g.containing fat-soluble vitamin without fat emulsions,absence of amino acid,fat emulsions or glucose,electrolyte concentration was unreasonable,the problematic TPN formulations between Jan.2010 and Mar.2011 were analyzed.The problems of prescriptions were classified statistically as well as the effect of pharmacist-physician communications.RESULTS：84 formulations were recorded among which the ratio of formulation using fat-soluble vitamin without fat emulsion was the highest（34.52%）,followed by the formulations without amino acid（19.05%） and high Ca2＋ concentration（10.71%）.After putting forward problems and communicating with physicians,physicians of 62 formulations（73.81%） accepted the pharmacists’ suggestions and revised the formulation.Physicians were easy to accept the pharmacists’ suggestions on drug compatibility,but had some doubts about electrolyte concentration,whether glucose,fat emulsion and amino acid added or not,and the proportion of them.CONCLUSION：There are problems in TNP formulations and pharmacist can communicate with physicians appropriately so as to promote the rationality of TPN.
