磁共振增强梯度回波T2^＊加权血管成像对结节性硬化症的诊断价值

摘　　要：目的探讨磁共振增强梯度回波T2^＊加权血管成像（enhanced gradient echo T2-star weighted angiography,ESWAN）序列对结性硬化症（tuberous sclerosis,TS）的诊断价值。方法同顾性分析经临床证实的9例TS患者的CT、常规MRI及ES-WAN的资料（9例均行颅脑CT、MRI平扫及ESWA扫描，其中CT增强2例，MRI增强的例）。结果CT表现为脑室管膜下或周围的多发钙化灶；MRI表现双侧大脑白质、皮层和室管膜下区多发结节样、斑片状异常信号影，室壁呈波浪状改变，T1WI呈低或等信号，T2WI为等或略高信号；ESWAN显示钙化性病灶呈斑点状低信号，灶周环绕斑片状高信号，而未钙化的病灶呈斑片状高信号，边界清楚。ESWAN与常规MRI、CT对病变结节的显示率具有显著性差异（X^2值分别为9.35和我13.75，P〈0.05）。结论ESWAN是常规MRI及CT诊断TS的有力补充。Objective To study the value of enhanced gradient echo T2-star weighted angiography（ESWAN） in the diagnosis of tuberous sclerosis （TS）. Methods CT and MR imaging data of 9 patients with TS confirmed clinically were analyzed retrospectively. All patients underwent spiral CT, MRI and ESWAN. Among them,2 cases and 3 cases underwent contrast enhanced CT and MRI re- spectively. Results CT showed multiple calcifications in subependyma of ventricle or around the ventricle. MRI showed abnormal node like or patch signals in the intracranial white matter, gray matter,anti subependymal areas, which were hypo-or iso-intense on ＂l＇~Wl,and iso or slight hyper intense on T,WI. The punctate calcifications were hypointense on ESWAN,and the lesions were sur rounded by patch hyperintensity . On the other hand , the non calcified lesions were patch hyperintense with definite margins . The displaying rates of the nodules by ESWAN , MRI and CT were of significant difference（X^2 value are 9.35 and 13.75 respectively ,P〈0. 005）. Conclusion ESWAN is of significant supplement to the diagnosis of TS.
