摘  要:目的:调查上海社区代谢综合征患者的膳食状况以及其对糖尿病营养治疗相关的知识、态度和行为(knowledge,atti-tude and practice,KAP)状况。方法:2007年9~12月采用分层随机抽样法,抽取上海市曹杨社区所辖的20岁以上居民3 000人进行代谢综合征筛查,将筛查出的368例代谢综合征患者作为调查对象,由接受过专门培训的调查员以面对面访谈方式进行糖尿病营养治疗KAP问卷调查,同时采用连续3 d膳食回顾法进行膳食调查。结果:368例代谢综合征患者中,糖尿病患者109例(29.6%,糖尿病组)、糖耐量异常或糖调节受损者154例(41.8%,糖尿病前期组)、糖耐量正常者105例(28.5%,糖耐量正常组)。与糖耐量正常组比较,糖尿病组和糖尿病前期组碳水化合物摄入比例较高(P<0.05),蛋白质摄入比例较低(P<0.05);3组患者维生素和矿物质摄入均未达到推荐摄入量(recommended nutrient intakes,RNI)。糖尿病营养治疗KAP调查显示,糖尿病组KAP总分显著高于糖尿病前期组与糖耐量正常组(P<0.05)。结论:应该采取有效措施以提高社区人群对糖尿病饮食及营养治疗知识的知晓率。Objective: To assess the diet and nutritional status of patients with metabolic syndrome living in urban communities in Shanghai, and to investigate their knowledge, attitude and practice (KAP) towards medical nutrition therapy for diabetes as well. Methods:Using stratified randomization, 3 000 residents aged more than 20 years from Caoyang community in Shanghai were sampled for relevant examination during July to September 2007. A total of 368 patients with metabolic syndrome (MS) were identified and interviewed face-to-face by specially trained investigators using questionnaire and a three-day dietary recall to evaluate their knowledge, attitude, and behavior towards dietary nutrition. Results: Among the 368 patients with MS, 109 (29.6 % ) were with diabetes, 154 (41.8 % ) were with impaired fasting glucose (IFG) or impaired glucose tolerance (IGT), and 105(28.5 %0 )were with normal glucose tolerance(NGT). The dietary survey showed that the diabetes group was significantly different from the IGT + IFG group and the NGT group in the ratio of carbohydrate and protein intake (P〈0.05). Significant difference was also found in the ratio of protein intake between the IGT + IFG group and the diabetes group (P〈0.05). All the three groups didn't meet the recommended nutrient intakes (RNI) in vitamin and mineral intakes. There were significant differ- ence in the total KAP points among the three groups (P〈0.05). The diabetes group got significantly higher total KAP points than the NGT group and the IGT + IFG group. (P〈0.05). Conclusions: Measures should be taken to improve the knowledge of diabetes nutrition in community populations.
