彩色多普勒超声对产前诊断脐带附着异常的临床价值

摘　　要：目的彩色多普勒超声在产前诊断脐带附着异常的临床价值评估。方法利用彩色多普勒超声对2009年8月至2012年7月间15793个孕妇在我院进行产前超声检查，其中中期妊娠为7028例，晚期妊娠为8765例，对其脐带附着处进行筛查，观察脐带附着部位、血流走向情况，追踪产后结果。结果7028例中期妊娠，彩色多普勒超声下全部可见其脐带附着位置及血流分布情况，8765例晚期妊娠，只有6319例可见脐带附着部位，约占72．1％，脐带附着部位的显示率晚期妊娠较中期妊娠明显降低；超声诊断球拍状胎盘共347例，占2．19％，帆状胎盘47例，占0．29％，帆状胎盘合并前置血管12例，占0．076％，经产后证实脐带边缘性附着为361例，帆状胎盘43例，帆状胎盘合并前置血管12例，漏诊10例，诊断准确率迭97％。结论彩色多普勒超声对产前诊断脐带附着异常具有诊断准确率高，简便而有效的方法，在临床诊治中具有一定指导意义，减少产科的并发症，降低胎儿围生期死亡率。Objiective To evaluate the color doppler ultrasound in prenatal diagnosis umbilical cord attached anomaly clinical value. Methods Using color doppler ultrasonography in August 2009-2012 between July 15793 pregnant women in our hospital antenatal ultrasound examination, including mid-term gestation for 7028 cases, late pregnancy for 8765 cases, the umbilical cord attached place screening, observe umbilical cord attached parts and blood flow to situation, tracking postpartum results. Results In the middle of the 7028 cases of pregnancy, color doppler ultrasound all visible under the umbilical cord attached location and blood flow distribution, 8765 cases of late pregnancy, only 6319 cases of visible umbilical cord attached parts, about 72.1%, umbilical cord at- tached parts of the display rate third trimester of pregnancy is middle decreased obviously; Ultrasonic diagnosis racket shape placenta common in 347 cases, accounting for 2.19%,Velamentous cord insertion 47 cases, account- ing for 0.29%, Velamentous cord insertion previa blood vessels merger in 12 cases, accounting for 0.076%, the postpartum conf＇n＇rned umbilical cord marginality attachment for 361 cases, Velamentous cord insertion 43 pa- tients with placenta,Velamentous cord insertion merger previa blood vessels in 12 cases, 10 cases of misdiagnosis and the diagnosis accuracy rate reaches 97%. Conclusion Color doppler ultrasonography in prenatal diagnosis umbilical cord attached anomaly has high diagnostic accuracy, simple and effective method, in the clinical diag- nosis and treatment has certain guiding significance in, reduce the obstetric complications, reduce the fetus mortality rate in the perinatal period.
