摘　　要：目的研究经胃镜放置空肠营养管后行肠内营养对改善急性重症胰腺炎(SAP)患者营养状况和预防感染的价值。方法对2010年1月-2011年12月期间收治的52例因SAP入院患者的临床资料进行回顾性分析,其中25例经胃镜放置空肠营养管进行早期肠内营养(EN)治疗,27例进行传统的全胃肠外营养(TPN)治疗,比较两种治疗方法对营养状况、预防感染及器官衰竭的影响。结果两组患者治疗前后体质量指数(BMI)、血清总蛋白和血清白蛋白的变化比较差异有统计学意义(P<0.05);CT评分、血白细胞计数治疗后差异均有统计学意义(P<0.05);治疗后器官衰竭和(或)局部并发症比较差异有统计学意义(P<0.05)。结论 EN是SAP营养支持的重要途径,能有效地预防感染和器官衰竭的发生,促进营养状态的恢复,与TPN相比具有明显优势。经胃镜放置空肠营养管是实施EN的有效可靠手段。Objective To study the value of endoscopic naso-jejunal feeding tube placement in improving the nutritional status and prevention of infection for patients with severe acute pancreatitis（SAP）.Methods We retrospectively analyzed the clinical data of 52 patients with SAP treated from January 2010 to December 2011.Twentyfive patients received enteral nutrition（EN） treatment with naso-jejunal feeding tube placement,while 27 underwent traditional total parenteral nutrition（TPN） therapy.The differences of nutritional status,infection and organ failure between EN and TPN were compared.Results The differences of BMI,total serum protein,and serum albumin changes before and after the treatment in both groups were statistically significant（P 0.05）.There were statistically significant differences（P 0.05） between two groups in CT score and white blood cell count after treatment（P 0.05）,and the same significant differences（P 0.05） also existed in organ failure and（or） local complications between the two groups after treatment. Conclusions Enteral nutrition plays an important role in SAP nutrition support,which is better than total parenteral nutrition because of better nutritional status,fewer infections and organ function failure.Endoscopic naso-jejunal feeding tube placement is an effective method in EN therapy.
