摘 要:目的:探讨胸、腹腔镜联合下微创手术治疗食管癌的近期疗效。方法:将本院行食管癌根治术的患者分为常规开胸手术组(30例)及胸、腹腔镜联合食管癌切除组(23例),观察两种手术方法的疗效及术后并发症的发生率。结果:胸、腹腔镜联合手术组手术时间比常规开胸手术组延长,但术中失血量、术后镇痛时间、住院时间及术后呼吸系统并发症均较常规手术组减少。淋巴结清扫数目及术后ICU监护时间无明显差异。结论:胸、腹腔镜联合行食管癌根治性切除术是安全可行的,具有创伤小、恢复快、术后并发症少、近期疗效满意等优点,值得临床推广。Objective:To explore the short-term effects of thoracoscopic and laparoscopic minimally invasive surgery to treat esophageal cancer.Methods:Based on radical surgery of esophageal cancer on patients in our hospital.They were divided into conventional thoracotomy group(n=30) and thoracoscopic combined with laparoscopic esophageal resection group(n=23).To observe the efficacy and complications of both of the methods.Results:Thoracoscopic and laparoscopic resection of esophageal carcinoma took longer operation time,but intraoperative blood loss,postoperative pain,hospitalization time and postoperative respiratory complications were all reduced to compare with the conventional surgery. Both the number of lymph node dissection and postoperative ICU monitoring time had no significant difference. Conclusion:Thoracoscopic and laparoscopic resection of esophageal carcinoma was technically feasible and safe.It had advantages of less trauma,faster recovering and less perioperative complications.It is worthy of clinical promotion.
