非小细胞肺癌组织中PTEN基因与Ki67的表达及临床意义

摘　　要：目的:Ki67在非小细胞肺癌和正常肺组织中的表达及其与患者预后的相关性。方法:收集67例有完整临床病理资料和随访资料的非小细胞肺癌组织及41例癌旁肺组织标本作为对照,采用免疫组织化学方法检测PTEN基因、Ki67蛋白的表达,并分析其与各临床病理指标及预后之间的相关性。结果:PTEN基因在非小细胞肺癌中阳性表达率为32.84%(22/67),而在癌旁肺组织中阳性表达率为82.97%,显著高于非小细胞癌组织(P<0.05)。Ki67在非小细胞肺癌组织中表达为76.12%,则显著高于癌旁肺组织27.31%(P<0.05)。肺癌组织中PTEN基因、Ki67的表达均与肺癌的临床分期、淋巴结转移密切相关(分别P<0.05),Kaplan-Meier生存曲线分析显示患者术后生存率PTEN阳性表达患者显著高于阴性患者(P<0.05),而Ki67阳性表达患者显著低于阴性患者(P<0.05)。PTEN蛋白阳性表达与Ki67阳性表达呈负相关(r=-0.239,P<0.05)。结论:PTEN蛋白表达的缺失及Ki67高表达与非小细胞肺癌的恶性侵袭、淋巴结转移有关。联合检测PTEN与Ki67的表达有助于判断非小细胞肺癌的预后。Objective： To study the expression of gene PTEN,Ki67 protein in non-small cell lung cancer（NSCLC） and normal lung tissue,and investigate their correlation with patient＇s survival.Methods： The protein expressions of PTEN and Ki67 were evaluated by immunohistochemistry in 67 NSCLC patients and 41 adjacent normal lung tissue.The associations with clinicopathologic data and prognosis of NSCLC were also analyzed.Results： The positive expression of PTEN was in 32.84% of NSCLC,while it＇s expression was 82.93% of 41 adjacent normal lung tissues,significantly higher than in tumor tissue.The expression of Ki67 in NSCLC tissue was significantly higher than in tumor surrounding lung tissue（P0.05）.Both of the expressions of PTEN and Ki67 were strongly correlated with clinical stage and lymph node metastasis（P0.05）,PTEN expression was negatively related to the overexpression of Ki67（r=-0.239,P0.05）.Kaplan-Meier analysis showed that down-regulated expression of PTEN was associated with lower survival rate of patients and down-regulated expression of Ki67 was associated with higher survival rate of patient after operation.Conclusion： The loss of PTEN expression and over expression of Ki67 play different roles in malignant invasion,lymph node metastasis and lower 4 years survival rate of NSCLC.patient.The loss of PTEN expression and the overexpression of Ki67 can estimate the poor prognosis of NSCLC.
