儿童急性白血病凝血指标的分析

摘　　要：目的探讨儿童急性白血病凝血变化及临床意义。方法 100例初发急性白血病患儿根据白血病类型分为急淋组58例和急非淋组42例,根据是否出血分为出血组63例和非出血组37例,检测各组及对照组凝血指标(PT、APTT、FIB、TT)和PLT数量,结果进行统计学分析。结果急淋组PT、APTT、TT、FIB和PLT分别为(14.77±1.91)s、(41.82±9.06)s、(16.78±1.92)s、(3.38±1.74)g/L、50.00×109/L,与对照组比较,APTT、FIB、PLT差异有显著性(P分别为0.023、0.027、0.007,<0.05);急非淋组分别为(17.48±3.12)s、(44.34±9.90)s、(17.37±3.62)s、(2.33±1.30)g/L,19.00×109/L,与对照组比较,PT、APTT、TT、PLT差异有显著性(P<0.01);急淋组与急非淋组相比,PT、FIB、PLT差异有显著性(P<0.01);出血组与非出血组比较,PLT、PT、FIB差异有显著性(P<0.01),APTT与TT差异无显著性(P=0.528、0.263)。结论联合检测凝血四项和血小板数能为儿童急性白血病分型和出血的发生提供实验室依据。Objective To discuss the changes and clinical significance of blood coagulation indicators in childhood a- cute leukemia. Methods The levels of APTI＇, PT,TT, FIB in serum were determined and Platelets （PLT） in 100 acute leukemia patients and compared with that in normal control group. Results A total of 58 case of, FIB and PLT are （ 14.77±1.91 ）s, （41.82±9.06）s, （16.78±1.92）s, （3.38±l.74）g/L,50.00x109/L. Compared with the control group,the differences of APTY,FIB and PLT in ALL patients were significant （P equal 0.023,0.027,0.007, 〈0.05）. 42 case of ANLL, PT,APTT TT,FIB and PLT are （17.48±3.12）s, （44.34±9.90）s, （17.37±3.62）s, （2.33± 1.30）g/L, 19.00x109/L. Compared with the normal control group,the differences of the levels of PT,APTr,and PLT in ANLL patients were significant （P 〈 0.01 ）;The difference of PT, FIB and PLT in ALL patients shows significance （P 〈 0.01 ） ,compared with ANLL patients. And the levels of PLT,PT and FIB were higher in patients with hemorrhage than nonhemorrhage （P 〈 0.01 ）,and there were no significant differences in APTT and TT （P equals 0.528 and 0.263）. Conclusion Combined detection of blood coagulation index and pit count can provide important experimental evi- dences in Acute Leukemia typing and bleeding occurrence.
